Hello; I am writing a dispute letter for the complaint for ID ***The patient was seen in our office on 01/31/and x-rays and a comprehensive periodontal exam was performedPatient was made aware of the periodontal disease he has Treatment was gone
over with the patient and patient did not want to do the treatment due to the fact his insurance will not cover the treatmentDr*** and the hygienist then went over a lesser treatment with the patients knowledge that they were not treating the disease just removing some of the build- up he has which still is not covered by insuranceThe patient did agree and signed a consent form for the treatment that was a full mouth debridement. The patients insurance did not pay for the x-rays that were taken and Dr*** did adjusted off the amount since the patient was upset about his insurance not helping with his procedureWe did explain to the patient what was covered and what is not, he has a very limited planHe did agree to the treatment and when he got home after the procedure was complete he has called several time to complain about he chargesWe went over it with him several times and he still is not understandingAt the time of his appointment he understood and agreed to treatmentHe does not have a balance with our office Sincerely The office of Dr*** ***

all balance. The lab cost is the amount she paid and we can't refund that since we incurred a cost and had to pay the lab bill. We never stated that we would refund her and funds just removed her balance.
