check via Certified Mail to the address we had on file, [redacted]. We confirmed via our bank records that [redacted] had cashed our check. It cleared our bank on April 13th, 2016. Tonia called [redacted] the afternoon of 4/14/16 and verified with her on the telephone that she had cashed the check. [redacted] stated that she was satisfied with the amount paid as agreed and that we were considered paid in full. Please see copy of the referenced documents attached. On Feb * 2013 I went to this location for routine eye exam an contact lenses. I have [redacted] insurance and this location is in network to provide services. They over charged me 46 dollars claiming that the fitting of contact lenses was not covered by the insurance. I informed the lady that I know my benefits and they are covered she alleged that since it was Sunday she had no way of calling and verifying so I had no choice but to overpay just in case the insurance did not cover. She said she will call back once the insurance paid if I was due a refund. She never called. I went in on 05/**/2013 and she said I was only due 20 dollar refund. she alleged the insurance did not pay everything. They are in contract meaning they have a negotiated rate that they must take and not balance bill the patient, and this is exactly what they did. They balanced billed me. If they did it to me I'm sure they do it to a lot of people that are not aware how insurance works. This is fraud.
