My staff completely disagrees with Mr. [redacted] 's claim that he indicated his worker's compensation claim was a Federal claim when he called for a consultation. In fact, it was noted int eh appointment that he was solely a worker's compensation claim. We do not take responsibility for the limitations in patients' insurance coverage as it is their responsibility to check with the providers.Since the amount in the dispute in not worth the time for further back and forth, I have authorized a refund to the patient in the amount of $125.00, which is the discounted amount he paid.I trust there will not no need for you to communicate with us any further. I am 100% sure that when I called to make my initial appointment that I advised this was a FEDERAL worker's comp claim I advised them again when I arrived for my appointment and again when they were discussing the surgery schedule After they submitted the request for surgery, the doctor said he did not want to participate in the federal worker's compensation program As *** ** *** would not pay their submitted bill unless they submitted certain paperwork, which they refused, they then sent me a notice advising that if I did not pay the bill within days, the account would be sent for collection My credit score is above and I didn't want this situation to adversely affect my score At the time the bill was paid, I advised them I would be reporting them to the Revdex.com They didn't care, they just wanted their money They also said that they could chose to participate or not participate in any program and they chose not to participate in the federal worker's comp program I will reiterate one last time, they were advised multiple times that it was a federal comp claim, they then refused to complete required documentation, and then they threatened collection [To assist us in bringing this matter to a close, we would like to know your view on the matter.]

patient's allegations.Very briefly, Mr. [redacted] called our clinic and informed my staff that he had Worker's Compensation (WC) coverage and was given an appointment. I examined him July 12, 2017 and diagnosed bilateral inguinal hernias. Mr. [redacted] did not verbalize to my staff of his status as a Federal Workers Compensation (FWC) claim. It was not until weeks later when we submitted the office consultation charge that we were notified that we are not participants in the (FWC ) program, although we are participants in almost all the Ohio WC programs.  Subsequent billing for the consultation to the FWC was therefore denied. To be clear, I did perform a complete examination and also authored a letter relating his work related injury to the diagnosed hernias. A copy is enclosed for your review.The standard consultation charge at our clinic is $300.00 which is what was billed to his FWC. Given the fact that the patient did not verbalize the FWC status of his claim and our lack of knowledge regarding our status with the entity, I authorized a reduction of the $300.00 charge to our self pay rate of $125.00, which is about a 60% discount. This was accepted by Mr. [redacted], as he has paid the discounted fee leaving a zero balance.Going forward we now make patients aware that even though we participate in some insurance programs, we may not be participants in the same insurance company's more narrow programs that have restricted physicians panels. Sincerly,David [redacted], MD FACSMedical Director
