The patient came to our office on 11/3/2015 for comprehensive initial oral exam and full-mouth oral images. During the exam the patients chief complaint was pain on the upper right area and the crown with a post continues to come out. The patient states...

the crown and post has come off three times since originally placed. This procedure was performed @[redacted] Dental in Mt. Pocono, and the patient states she did NOT like their dental office, This tooth had a recurrent abscess and root caries. A diagnosis was made that the tooth was unrestoreable and required extraction.A consultation was scheduled and completed on 11/11/15. During the consultation the patient was informed that she had the following dental conditions: tooth decay, missing teeth, and defective restorations. The treatment plan reviewed with the patient was to consist of the following:
1) Extraction #42) Root canal treatment #143) Cleaning4) Fillings(4)5) Post & Core6) Crowns(4).
Patient understood the need for the recommended and necessary treatment and financial arrangements were discussed and reviewed with her. The patient accepted the proposed treatment(see attached form) and elected to finance her treatment via [redacted].
On 11/19/15 patient arrived for scheduled appointment to extract #4 and root canal treatment.  Patient was scheduled for next dental appointment on 11/25/15. On 11/25/15 patient cancelled appointment via phone. On 12/4/15/our office responded to a complaint filed by [redacted](see enclosed).
On 12/9/15 [redacted] informed our office they received our fax regarding dispute. Our office called the patient and wanted the patient to come to our office to discuss the patients concerns with NOT proceeding with the recommended necessary treatment plan. The patient stated the following excuses :1) She didn't feel confident,2) It didn't matter because she couldn't afford the treatment,3) She had no transportation, and4) She had to get off the phone.
On 12/15/15 [redacted] issued patient a refund of $5904 for services not performed (copy enclosed.
