They provide service other than we requested without telling us prior to it.
On 01/19/2012, my son, [redacted] (4Y old), went to see Dr. [redacted], of [redacted] Family Dentistry, for his 1st periodic cleaning. On that service Dr. [redacted] told us that my son has cavity and need to be filled. However he doesn't feel comfortable to do it in his facility due to lack of proper equipment for such young age. So we request a referral towards Dr. [redacted], of [redacted] , DDS, as she is a pediatric dentist.

So on 01/26/2012, we saw Dr. ** for oral exam., specifically on purpose of the cavity assessment. But it turned out they file towards insurance under 'Periodic Oral Exam', hence exhausting our annual limit for periodic exam (our dental policy only cover 2x per year).

I found out about this recently and on 07/09/2012, I called [redacted] , DDS and complaint about this. I then was told then what they can do is to change it to 'Comprehensive Oral Exam.' instead, but most likely both comp. & periodic oral exam are sharing the same annual limit. So I called my insurance carrier, [redacted], and got confirmed that is indeed the case. However [redacted] told me that the provider could have filed under 'Problem Focus Exam.', which is actually the case as we went there specifically for filling the cavity.

So I called [redacted] , DDS again and asked them to change the filing to 'Problem Focus Exam.', but they refuse as they stated they always performed comprehensive oral exam. whenever they receive a referral. When I complained why I wasn't informed prior to it, they stated that this is their policy and don't tell the parents unless they ask. How could I asked if I didn't even know that they are performing service that I didn't request? At the end, the still refuse to re-file.

So basically, we went there requesting for them to performed filling on my son's cavity and was under impression that the exam. they are performing is specifically related for the filling. But instead, they are performing a service that I didn't request, didn't want, and didn't need and NOT telling me prior hand to it.

Because of that, my son is running out of his annual periodic oral exam. annual coverage.
