This patient signed our payment policies when she established care with our doctor on 06/12/2015, which states that a $fee will be assessed if a patient cancels an appointment without providing at least hours notice, or does not show up to a scheduled appointment This patient cancelled her appointment on 10/07/without providing at least hours notice, which incurred a $fee She then did not show up to an appointment on 10/27/2016, which incurred a second $fee She called our Billing department on 10/27/2016, and stated that she "was never informed of the $fees." Our biller left a message for the patient on 10/31/16, and the patient returned the call and spoke to our biller that same day Our Billing Department is authorized to waive only one (1) fee per year, so the patient's first $fee was waived at that time The patient became angry that the second fee would not be waived by our Billing Department She yelled at our employee, and used excessive profanity She threatened to disparage the doctor's reputation to all of her friends This was relayed to the doctor Per the doctor's instructions, the second fee was not waived, and the patient was discharged from our practice due to "professional discord and rudeness to the staff." Since the remaining balance has not been paid, it has been turned over to our Collection Service Of note, this service DOES NOT report outstanding balances to the Credit Bureau if the total amount is under $75.00, which is the case with this patient This account will NOT be reported to the Credit Bureau
