Review: This Doctor's office has billed me and my insurance for a doctor's office visit that didn't take place. I did not make an appointment. I haven't even contacted this office by phone.

The only doctor's visit I have ever had in the Los Alamitos area took place 5 years ago. And I'm not certain that this was the office.

As far as I am concerned an error of this magnitude is insurance fraud. I did not have an appointment on 6/3/14 as the bill states.Desired Settlement: I want a written explanation of how they came to bill me for a 6/3/14 appointment that did not take place

I want a written explanation of how they got my insurance information

I want my insurance company refunded and written acknowledgment of when they start and finish this procedure.

I want a bill showing a zero amount due from me.

I want a letter signed by the doctor that I can send to the credit bureaus stating admitting that they made this error - should it ever become necessary.

I want a written assurance that they have removed all of my insurance information from their records.
