Review: I visited this provider on at least four occasions. I've asked for an accurate and complete billing statement of my visits with co pays and have not received the requested. I have made multiple requests including calls on 7/16, 7/4, 6/22, 6/3, 5/19 and 4/29. I did receive only one billing statement but it is not complete/accurate and only includes two visits (3/24/15 and 4/2/15). I saw this provider at least once alone and at least three with my son. One visit included a cash payment of $20; the provider insured me that my statement would reflect this payment as well, but it does not and only shows two co payments made with checks. I saw this provider on these dates 3/18 (check# 460), 3/24 (check# 461), and 4/2 (check# [redacted]). There was at least one other visit with cash pay, the date of this visit is possibly 4/17 or 4/24; my husband and son were also present when I made a $20 cash co payment. I believe that the 4/24 visit was canceled however. I feel that I have made a great effort to retrieve accurate billing information and have not.Desired Settlement: I would like complete statements including all visits and payments made, including the $20 cash co payment. The provider should have an accurate record of our visits and payments.
