On 10/18/2011 I visited the doctor. For this visit the insurance company says I am to pay $43.57. The Doctor office has sent me a bill for $91.43. Claim Number: [redacted] with United Health Care states that Provider Billed $150.00, Payments and Adjustments -$106.43, and that my responsibility is for $43.57. All attempts to have this corrected has been denied by the doctors office. I intend to send a money order for $43.57 completing my obligation to them.
