experienced The actual balance on the account was $I have contact our Collection Agency and instructed them to forgive THE PATIENT’s balance in order to put this matter to restTo explain the situation: THE PATIENT was seen only one time in our office as an emergency patient on 9/27/He had two emergency surgical extractions and necessary x-raysTHE PATIENT was informed before any work was performed during our medical history review that if his insurance did not cover the procedure that he personally would be responsible for the chargesHe agreed and signed our consent form It is standard procedure that all new patients are required to show ID along with a copy of their insurance cardWe made an exception in this case, THE PATIENT had neither on him and because he was in such pain, we proceeded on his wordTHE PATIENT never returned to the office after that, not even for suture removalWhen the claim was submitted, all information submitted on the claim was the information that THE PATIENT had provided at original visitThe original claim for payment was sent on the date of service 9/27/THE PATIENT did not have dental coverage through Independent Health nor was he eligible for *** coverage on 9/27/Several time we tried to contact THE PATIENT and verify the information that he provide but patient did not return our calls It was not until Octthat we received written notification from *** that he was not eligible for Dental *** Coverage. Eligibility was then again attempted to be established on 10/3/and again status came back ineligibleOnce coverage was denied, the claim was then resubmitted several times to see if there were any errors in the submission of the formClaim was resent with final explanations and x-rays on 5/17/and again on 7/12/Coverage was still denied- patient was not eligible at time of serviceUp until 5/17/we had tried to reach THE PATIENT to resolve this matter and to obtain any further information that might help us in processing his claimWe received only silenceWe have no record of the patient contacting us, nor of making any arrangements with our billing department. We started sending THE PATIENT statement beginning on 6/1/We asked to patient to contact us if there were any problems in writing on the statements Again we did not receive any feedback from THE PATIENT. Statements were sent monthly until 9/4/at which time we turned his account over to our Collection AgencyWe sent him over billing statements and did not hear anything back from THE PATIENT until we received this complaint from your organizationThis week we have tried to contact *** to request written explanation but they can only go back two years to verify eligibility on a patient*** informed me to try and contact the local department of Social Services, however, I will need a written consent from THE PATIENT in order to be HIPPA compliant in order to have that information releasedJudging from the response we have had in the past from this patient, unfortunately that leaves my hands tied with this situationThe actual balance on the account was $I have contact our Collection Agency and instructed them to forgive THE PATIENT’s balance in order to put this matter to restAlso to forgive any charges that they may have added for collection Joseph H*** DDS
