Dear Revdex.com, I have practiced for years, of which I have never been in network provider for any insurance companyNeither myself nor anyone working for me ever tells a patient that we are in networkI am out of network for the majority of my patientsWe do a treatment plan with my fees
that does break down an estimated amount that the insurance company should pay based on the information that the insurance company provides to usThe treatment plan is signed by an employee of mine and the patientA copy is given to the patient. Above where the patient signs on the treatment plan is a statement that they read and then sign: Case Notes: I understand that treatment recommended is determined by what the doctor has diagnosed as needed, this is not always compatible with what the insurance paysInsurance quotes are estimates only, and are based solely on information given to us by your insurance companyWe will be happy to file your insurance for you as a courtesy, however you are responsible for any unpaid balance as we can only give an approximate of what your insurance will pay, The following fees are good for days The following treatment has been explained to me, and I understand what has been recommendedI also understand that healthcare can occasionally have unforeseen events which could require additional treatment.Also on the new patient form ( which the patient reads and signs)" I understand that my insurance is an an agreement between by insurance company and meI also understand that I am responsible for the balance of my dental account regardless of my insurance I understand that every insurance policy is different and that it is my responsibility to understand my insurance policy exclusions and limitations. It is very unfortunate when an insurance company does not pay as expected based on the information that the patient and the insurance gives usWe tried numerous times to contact this patient prior to turning over to collectionHe chose not to respond until he was turned overI would be more than happy to give you a copy of all signed documents if you can present me with a signed release from the patient. That is not true. I contacted them through email as well as through the phone. Yes, they don't say they are in- network. They are deceptive and say, yes we accept your insurance without notifying you that you are out-of-network.

Review: led me believe that my insurance was in network when it wasn't. Misrepresentation because they never once told me I was out of network or I would have never used their services.Desired Settlement: I've already paid well over $200 to have two cavities filled. And now they want $355 more. $500+ for fillings is ridiculous. I do not want to pay anything more than I already have.
