This doctor coded my husband's wellness check incorrectly. He was there for a routine check-up/wellness visit approved by our insurance Co as "no cost share" preventive care under the ACA. He has no health problems, takes no prescriptions, is a healthy 38 yr old man who came in to get vitals, lipids, sugar tested per our Ins Co. He left with no diagnosis, meds, not even the bloodwork was completed...just vitals & talk briefly w/ Dr. I called to have them re-check the appt notes so they could correct the coding. They left a voicemail stating they would not make the correction. I called back for an explanation and was promised a return call from Office Mrg after discussing with Dr. To date, I have never received the explanation, and they have been ignoring my polite voicemails for two months while continuing to bill us. We still do not know why they are standing by the improper coding. We both asked for and received a routine, preventive-care wellness visit. It was plainly requested at appt scheduling, as well as the moment the Dr. entered the exam room. As stated above, this is also what we received. There is no justification for coding otherwise and especially not for disrespectfully ignoring a patient trying to resolve a billing issue w/ no explanation provided.

Desired Outcome

Billing Adjustment I would like them to correct their billing to the CPT corresponding to what we requested and received: an annual routine, preventive-care wellness visit mandated by ACA for adults. As such, they would still be paid the same amount, however it would come from BCBS and not us. This is the proper thing to do, according to the facts of the matter....we are not requesting any special treatment.

We understand the concern expressed by Mrs. regarding Mr. January visit. Upon initial review of the inquiry, it was interrupted a medical discussion and diagnosis was recorded. Our appointment record reflects a new patient visit was requested with no reference of a preventive visit. Unfortunately, terminology and/or perception of an establishment with a new physician versus a new patient preventive visit can be confused, particularly if during the exam a patient discusses a medical condition with the physician. Mr. did mention he has abdominal discomfort when consuming gluten or high fiber foods and a celiac disease panel along with other labs including age appropriate screening labs were recommended and a lab order was generated. The visit was coded as an abdominal pain workup. Our laboratory records indicate the patient did not present for the lab services. During the visit it was determined a Tdap vaccination was warranted due an infant in the home. Upon further review, we have converted the visit to a preventative exam. We do have record of the office manager unsuccessfully attempting to contact Mrs. twice with Mrs. returning the calls and unsuccessfully reaching the manager. The manager acknowledges she has not reached out to Mrs. again due our shift to COVID-19 demands. We apologize for the confusion regarding the above mentioned interpretation and lack of communication follow through.

I am assuming they will re-submit to insurance, as they state above it has been converted to preventive exam coding. As long as they do convert the coding to preventive exam as stated, I accept and appreciate the response from the business. Thank you for resolving!
