Complaint: [redacted] I am rejecting this response because: Regards, [redacted] [To assist us in bringing this matter to a close, the consumer must give a reason why they are rejecting the responseIf the consumer does not provide a reason if he complaint will be closed Administratively Resolved]I do no in any way accept this response If it was a matter of getting power of attorney, why did the receptionist who I spoke to on numerous occassions just say that I would've have presented it I handle all of my husband medical issues He approves (according to HIPAA) at all of his doctors' officesDon't dentists have the same thing (HIPAA) that is? Any other doctor my husband has signed that they are allowed to discuss his medical issues (chart) because of the HIPAA In any case, I believe that is on the dentist responsibility to say, "we need a power of attorney or a signed HIPAA" is it not?Again, IT IS NOT UP TO YOU OR YOUR STAFF TO MAKE MEDICAL DECISIONS ON OUR BEHALF BY DECIDING TO DO WHATEVER TYPE CLEANING YOU WANT THAT IS OUR DECISION YOU HAVE NO LEGAL AUTHORITY TO MAKE MEDICAL DECISIONS ON OUR BEHALF EITHER!!!!!!!!!!!!!!!!!!!!!!!!!

February 2, 2016 Regarding Patient: [redacted] Findings on 11/5/15: Patient had not had dental cleaning in 1.5 years. Radiographs show gross calculus build up both supra and subgingival. Patient informed a full mouth debridement would be required to remove tartar from teeth. Patient agreed to...

procedure before the procedure was performed. All tartar was removed with a piezoelectric instrument and hand curettage. Patient instructed to avoid spicy foods and rinse with warm salt water for a few days after procedure. Take Ibuprofen as needed for pain. Patient instructed that in order to avoid deep debridement in the future to please return to clinic at regular six months intervals rather than 1.5 year plus intervals. Hygienist performed appropriate procedure and it would have been unethical and malpractice on her part to perform any other procedure other than full mouth debridement. Note:  Mr [redacted] has not called Kirk * K[redacted], DDS, PC to talk about his case at any time. Only his wife has called and she has not provided office with proof of power of attorney. To speak to Mr [redacted]'s wife regarding his case would be a gross violation of HIPAA without proof of power of attorney. All interactions with Ms [redacted] have been recorded, in detail, within the medical record. Enclosed: Patient Ledger, all treatment notes regarding patient, procedure fee statement and radiographs (as per signed release by Mr [redacted] authorizing release of his medical information.
