Letter dated 11-13-is incorrectMy appointment on January I informed [redacted] at the front office that my insurance had changedMy new Insurance was [redacted] At that time I asked if they took that InsuranceShe replied "yes"As for the other dates of January 16th, February 6th & February 20th she stated as being freeWhy was I charged $which is now on my credit report? I feel that in when I was seeing her for my thyroid and diabetesThat she was having me come every two weeksBecause my insurance was paying 100% of her services which I was informed by previous office employeesMy new endocrinologist has me coming every two to three monthsNot every two weeks and my thyroid level and diabetes are still not where they need to be at this timeThe only thing that I want to happen is $come off my credit reportIf I was told at the time of service that my insurance was out of networkI would of searched for a new endocrinologist in which I have done so.Thanks and I appreciate your help in this matter [redacted] ***

To Whom It May Concern;*** *** was seen frequently as she was severely hypothyroid and her thyroid level was in the range that could lead to myxedema comaHer medication needed to be adjusted and levels had to be monitored after the adjustments were made so that thyroid levels were in the high
ranges suppress thyroid cancer growthHer diabetes was poorly controlled and her glucose levels had to be closely monitored as medications were added.The balance for *** *** was from date of service January 9, which was billed to her *** *** ***e and applied to her deductibleAny date after January 9, was billed to *** and the claims were denied as out of networkThese dates of service were never billed to the patient as a courtesy of DrSkaff the patient was seen for free by DrSkaff on three dates January 16th, February 6th, and February 20th.Sincerely,Lee Ann Skaff, M.D

Letter dated 11-13-2017 is incorrect. My appointment on January 9. I informed [redacted] at the front office that my insurance had changed. My new Insurance was [redacted]. At that time I asked if they took that Insurance. She replied "yes". As for the other dates of January 16th, February 6th & February 20th she stated as being free. Why was I charged $306 which is now on my credit report? I feel that in 2016 when I was seeing her for my thyroid and diabetes. That she was having me come every two weeks. Because my insurance was paying 100% of her services which I was informed by previous office employees. My new endocrinologist has me coming every two to three months. Not every two weeks and my thyroid level and diabetes are still not where they need to be at this time. The only thing that I want to happen is $306 come off my credit report. If I was told at the time of service that my insurance was out of network. I would of searched for a new endocrinologist in which I have done so.Thanks and I appreciate your help in this matter.
