[redacted]  I do not agree with the s[redacted]ment from Dr. Malcolm Walter's Jr. office. I was never told and it is not in my contract that my service would be terminated upon notification from an orthodontist that I contracted with him or her for orthodontic services. I notified his office verbally on my intent to terminate services for myself as of 08/31/2016 and his office terminated the EFT as of 08/31/2016, so, I informed his office verbally that my daughter services would be terminated as of 09/30/2016 . The lady in his billing department s[redacted]d, she would put a note on Jacari's account to remember to terminate the EFT as of 09/30/2016, so, no withdrawal would be taken out of my account for October and thus forward. His office fell to terminate the EFT as of 09/30/2016 and an EFT was taking from my account in the amount of $150.00 on 09/30/2016 for October 2016 services. That is the amount I am requesting a refund for. Please ask Dr. Walter's office to submit the signed agreement to Revdex.com. Sincerely, [redacted] D [redacted]
