Review: I have not been able to get the office staff to submit the paper work and xrays that are needed for me to get reimbursed for a crown replacement and a regular check up. These services took place in Sept. and Oct. of 2012. I have even spoken personally with the dentist to inform him of the situation and also of 2 appointments that no one was there when I showed up. Dr. Malizia assured me that he would personally take care of the matter, which he did not. I have called the office staff several times since then, and was assured that the papers would be submitted. My last calls were to ask them to give me the imformation and xrays, and I would submit them. I was told they would be ready on two diffenent dates, and as of this morning they were not ready when I went to pick them up. I have never had any complaints with my dental service from Dr. Malizia,except for him not following through with his promise. I am tired of calling their office and actually going into the office and not getting anything accomplished over the last six months.Desired Settlement: I would like to have my necessary paper work to submit my claims, as promised. I want other people to know about these problems that I have had if they are looking for a dentist. Also, I would hope that Dr. Malizia would be made aware of other people in this situation. I find it hard to believe that I am the only one this has happened to. I have always been respectful in my dealings with the staff, and I don't feel that I deserve this.

This email is to inform you that I have resolved my issues with Dr.
[redacted] Malizia's Dental office. I had called the office and told them
that I would be in to pick up my forms and x-rays. After numerous
promises to have the information ready or sent to me, I finally went
into the office and told [redacted] that I would wait in the
waiting room until she had it ready. I told her exactly what I
needed, again, and she still forgot the x-rays. I had to remind her
again that that was the reason that I had originally asked her to
submit the paper work, because I did not have the x-rays. I did get
an apology from [redacted].
My issues have been resolved. I would like to know why I was
treated this way. I hope that no one else has the same problems. As
previously stated, I had no problems with my actual dental care for
all the many years that I was a patient of Dr. Malizia. Thank you for
your help in this matter.
