I've been a patient of Dr Burchman's since 2006. Unbeknownst to me, he had brought on another Dentist. This was first brought to my attention when the office called to confirm my emergency apt for the following day. Since I was having pain, I didn't want to push the apt further out to see Dr Burchman although I was never told when making the apt that I would not be seeing Burchman. I don't even recall the name of this other dentist but I wasn't satisfied with my visit with her. She barely looked in my mouth. In the past, Burchman would have also resolved the issue at this visit. Apparently things were being handled differently than in the past. Apparently it was not in the best interest of the patient. I reluctantly agreed to come back to see Burchman not only for a cleaning but also to fix the pain in my mouth a month later. At my next visit, I expressed my dissatisfaction with this other dentist and if I am going to see a new dentist, I will find one closer to home. (I moved over an hour away in the last 2 years.). At this next visit with Burchman, I was advised that not enough time was scheduled to fix the problem that I'd been having and was told to ask him. He did agree to fill the cavity. At some point, he took an X-ray that was not covered by my insurance. I was never advised that it wouldn't be covered. I've expressed on numerous occasions that I don't want to have services that are not covered. I did speak with my insurance and another type of X-ray could have been done that was covered. I also want to add that I would've never have needed a 2nd opinion had I not seen this other dentist. I also don't recall him filling 2 cavities. I only recall the one I was having a problem with. I paid my coinsurance for the cavities which I expected due to being familiar with my benefits. I have not paid for the X-ray that was not covered due to the fact that a different X-ray would have been covered and having expressed not wanting to have non-covered services.

To Whom It May Concern:

Both necessary x-rays and Intraoral photos were taken on patient at her 7/9/2019 visit to see Dr. Mel Burchman, during which an exam and cleaning were performed along with diagnosis and treatment of composite Fillings on two teeth that were causing the patient discomfort. The X-rays and intraoral photos were necessary for proper diagnosis and treatment of the patient. We do not treat our patients based on their insurance, we treat them according to what is needed and we are in no way at fault or responsible for needing X-rays or photos in order to diagnose our patients. The patient has an obligation to her insurance company and the EOB we received from them for the DOS 7/9/2019 indicates that the patient is responsible for the amount she has denied our office payment for, part of which is from the fillings. We will be happy to provide these documents upon request, If necessary, in order to pre spending any more unnecessary time on this matter. As the patient stated, she has contacted the office on numerous occasions and spoken with several staff members in order to dispute her balance. We have cancelled the patient's future appointments as per her written request.

To Whom It May Concern:

As I was preparing the necessary documents to confirm the legitimate charge that the patient is disputing, I noticed that the amount due from the patient was a total of $96.60 according to the insurance company. She paid $49.20 at her visit on 7/9/2019 and sent a check for $27.40 on 10/20/2019. That means her balance due should be $20 and not $32. I researched further and found that the participating insurance adjustment of $12 had not been posted for the DOS 6/11/2019. The amount due has been adjusted to reflect this difference. I have also attached the explanation of benefits from the patient's insurance company confirming that the patient is responsible for the charge. Thank you for your time.
