abalance of $120.16 patient responsibility because there was no referraltherefore goes to her deductible. (See attached) Four statements wereissued to Ms. [redacted] in a four month span, never hearing one word fromher, also leaving phone messages regarding her balance due. She alsocame in on 5/11/15 for a follow-up visit and xrays. Collected a $40.00 copayand a claim for $150.00 was filed on 8/11/15. That claim was deniedfor timely filing (See attached) and written off and Ms. [redacted] was notcharged.This was sent to collection on 2/17/16 after 10 months of never hearingfrom her and the collection agency finally got her attention!!! If she wouldhave reached out to us this could have all been prevented, but bottom lineshe still owes $120.16.
