After much review, we have reviewed our customer's experience and verified all options were appropriately presented to her. Our customer was appropriately informed and notified of [redacted]...

[redacted] leaving our practice. She was provided with his new contact information and address when she contacted us for continued care on July 17th, 2017. Unfortunately, we are unable to answer for the issues she experienced contacting [redacted]'s new office location, as this is not a business associated with our current practice. The services rendered to our customer by [redacted] were unique to this physician and not able to be further addressed by our staff. Otherwise we would have been honored to provide continued care for her needs. Our clinical manager and office manager assessed our customer's care while she was present in the office on July 18th, 2017. We confirmed her best option would be to follow up with [redacted] at his new practice. When she voiced issues contacting [redacted]'s new practice, we made the attempt to coordinate her care before she left our office. We successfully contacted [redacted]'s new practice and coordinated an appointment for her before she left our office. We confirmed she had the phone number and address for [redacted]'s new practice, ensuring she was given the direct contact information for our clinical and office manager should she have future needs at our office. To our knowledge, the customer is now continuing her care for follow up with [redacted] at his new practice. We have not been contacted by the customer since July 18th, 2017 and resolved her concerns to the best of our ability in consideration of the fact that [redacted] is no longer with our practice. Should she require additional services with our practice, we will do all we can to provide her with exceptional service and care. It is our core value to serve others and continually seek to understand the needs of those who depend on us.
