August 10, 2017To Whom It May Concern,This letter is in response to your statement regarding account number [redacted] complaint made by [redacted] [redacted] chose with withhold insurance information from our organization until 06/05/ [redacted] supplied us with his insurance information of Medicare-PAHe was informed that we need all insurance packages that would be covering his medical claims and he informed us that Medicare was his only insurance packageAt no point in time did [redacted] disclose that Access was his secondary [redacted] also signed a patient consent form verifying that Medicare was his only insurance package We had in his medical recordAt no point in time did [redacted] contact us to discuss his deductible balance that he received due to his Medicare insurance only covering 80% ofhis claims.On 06/05/ [redacted] came into the office for his scheduled appointmentWhen he was checked in that day he was extremely rude to the front staff and made one nurse very uncomfortableOnce his physician entered the exam room, he stated very aggressively to the physician that he was angry that we sent him a bill for his deductibleHe was told that we were first informed of this issue by a phone call from the Department of Health who informed us that [redacted] opened a case with them regarding this billing issueAt that time Isabel entered the room to discuss his billing issue with himHe was told that he withheld this information from us and never supplied us with a secondary insurance and the insurance cardAt that time he was again asked for the card which was given to the billing department and his claims for his deductibles were sent to Access that very moment Although our organization informed him that it is his responsibility to disclose all necessary information to his doctors offices and that at that very moment we submitted his balances to his secondary insurance, he continued to use inappropriate language and behavior to the staff, the physicians and even the owner of the organizationHe was told while in the room with his physician, Isabel from billing and the owner of the organization that if he continued with his inappropriate behavior to staff that he would need to find a new primary care doctor [redacted] continued to act inappropriately and was informed of his dismissalHe was given a copy of his dismissal letter and one was sent to his home as wellAs we stated to [redacted] during his last office visit, once PA Access has processed all of his claims successfully, [redacted] will be notified and his credit of $will be refunded.It is our office policy that we do not allow patient in our organization who act inappropriately, use offensive language, utilize inappropriate ways of communication or purposefully withhold necessary informationEach and every patient is aware of these policies when they sign a consent formIf you have any questions or concerns please call us during regular business hours.Respectfully,Matilde SMD

desk where he was asked to present all insurance plans including any ***.He only disclosed and supplied us with his *** *** *** *** cardIn addition, he signed our medical treatment agreement where he veri?es and con?rms that he has given us copies of all insurance plans to which his bills are to be sentHe never noti?ed us nor supplied us with any other insurance informationbesides his *** insurance at this time. *** *** later received a bill for his deductible that his insurance plan states he is responsible forWhen we were able to get in touch with *** *** by telephone, he demanded that we inappropriately change the billing information that was sent to his insurance on the claim so his *** insurance would not charge a deductibleWhen *** *** was told that it is against the law to alter and/or sent a medical bill with information and that we would not do so, *** *** became very angry and used inappropriate language as well as threatening remarksIt was only at this time when *** *** stated over the phone “well if you want to be paid you better charge my Pennsylvania *** card”When he was asked why he didn’t present this secondary insurance when he was ?rst presented in our of?ce, he had no answer as to whyHe also refused to give any information over the telephone as well as bring in any card for its to verify and copy into his chart for billing purposes. To this date, we have not heard or received any correspondence whatsoever from any congressman*** *** was instructed that he did withhold insurance information from us, that he would have to supply us with this information so the appropriate insurance could be billedAs to this date *** *** still has made no effort to supply us with this informationOnce we receive this information, PA *** will be receiving a bill for his deductible.** *** has since been permanently dismissed from our of?ce due to his inappropriate language and threatening remarks he made not only to our billing department but also due to the uncomfortable treatment towards but stuff and physicians.If you have any other questions regarding *** ***’s Medical Treatment Agreement in which he signed con?rming all insurance information is correct, please call our of?ce during business hours.Thank you,Sotomayor Medical Practice, PC
