Patient *** *** was referred to our Periodontal and Implant specialty office by her general dentist for an evaluation on 07/**/After a careful and thorough radiographic and clinical examination, it was determined that *** ***s was generalized moderate chronic periodontal disease
Upon completion of the first phase of therapy, which included full mouth scaling and root planing, it was further determined that *** ***s disease remained and would be best addressed through a procedure known as osseous or periodontal surgeryFurthermore, due to localized vertical defects in the jaw bone surrounding select teeth in *** ***s mouth, it was determined that those areas could best benefit from a bone grafting procedure which would help the remaining bone regenerate and further support the teeth that had been damaged from the periodontal disease process as well as prevent future bone loss and loss of teethThese surg ical procedures were performed without incident on 11/**/Appropriate follow up was conducted and all phases of healing were normal*** *** has not returned to the office since 11/**/15. Regarding payments and billing for this procedure, a dental insurance pre-determination was obtained for all necessary surgical procedures, including the periodontal osseous surgery and all teeth that would require bone graftingThe procedures were approved by *** ***s dental insurance carrier, and were completed on 11/**/Unfortunately, the procedures were not covered entirely, as previously expected, due to the maximum yearly allowance limit having been reachedThis may have been due to the time lag between the insurance pre-determination which was processed on 08/**/and the date of the performed procedures on 11/**/during which *** *** may have had other dental services provided elsewhere which were paid for by her dental insurance carrierThis resulted in *** *** being help responsible for any remaining financial balances related to the performed procedures
