This is in response to the above complaint At this time we are writing off any balance due that their insurance company denied payment on after receiving a letter from his Insurance company dated January 15th stating that his plan does not cover vision services or vision materialsMr [redacted] should be aware that his insurance does not cover these services as I am sure his insurance company will provide him a list of coveragesIn the future he will have to pay in full for all Vision services rendered being the fact his Tricare Insurance does not have Vision coverage At this time his and his wife [redacted] accounts now show a zero balance and there is no outstanding balance due from them.If I may be of further assistance, please contact me at 317-924-X ***.Sincerely, [redacted] Director of OperationsDrTavel Family Eye Care317-924-X ***

Very disappointed in DrTavel for their delivery and help to resolve my issueMy son has now waited for weeks for his glasses to be deliveredI paid extra to have them shipped to my home and of course they were shipped to the kokomo, indiana facility( hour drive one way for exam to begin with) My first call to them, I was informed that they had them and would send them from thereNowweeks later I am informed that they shipped them back to the lab to be sent to meI asked to cancel my order and of course was told I can not do thatNow I just sit and wait for them to magically show up at my house while my son can't see and has no glasses at all When I am told something will be delivered in to weeks I expect them to be hereIt would be one thing if it wasn't glasses that people need to function on a daily basisI will never step foot inside a DrTavel again nor will I ever do business with them EVER!!!!!

I made an appointment for my daughter, a college student, weeks ago for Sat, November @ noonWe brought her home from college specifically for this appointment so she would have her glasses delivered over Thanksgiving breakI get a call @ 10:the morning of the appointment saying, "Ummmm yeah we don't know how she slipped through the cracks, but our Dr who's just out of school is waiting on her insurance credentialing so we can't see your daughter." This is TOTALLY unacceptable for a company who works with insurance all day EVERY dayNow not only will my daughter not have her glasses, but I have the added expense of bringing her home for an appointment that they canceled due to their ineptnessHorrible, horrible customer serviceMight I add we have been customers there for many years and the decline in that store is evidentWe will never go back

I have been going to DrTavels here in Terre Haute for ten years and the last three I have had issues with the productsBy that, I mean that my bifocals keep coming to me incorrectly fittedLast time two years ago it took three times to correct themThis year it took four times and now they tell me they won't fix them againI am Medicaid and they tell me I am stuck with themMedicaid says I don't have to accept defective merchandise so I filed a grievanceNow I contacted corporate to see what gets resolvedBTW my mother-in-laws are defective as well and sent back three times and still not fixedBuyer bewareI do like Dr. *** though

They will refuse to give your previous prescriptions and medical record's to you which is a direct violation of your right's as a citizen. I called them multiple times to do this and have been refused every single time. I'm going to seek legal action because they make a ridiculous amount of money ripping people off like this. Find an eye doctor that cares more about their patient then their patient's money.

Mr. [redacted] brought his son [redacted] in to our Richmond Dr. Tavel office on March 5, 2012 for an eye examination and glasses. At that time they presented [redacted]'s [redacted] insurance card to be billed for the services and materials. We received a denial from [redacted] Stating [redacted] had...

other insurance that needed to be billed first as the primary insurance.  We sent a letter requesting this information to Mr. [redacted] at [redacted]. We obtained this information from the patient registration form that Mr. [redacted] had filled out and signed on March 5, 2012.  I have attached a copy of the form and one from 2010 that both had the same address on as [redacted] had been a previous patient. We subsequently sent a 2nd and a 3rd and final notice to that same address and did not receive any responses.  I have attached copies of the letters that were sent. In September of 2012 the account was turned over to collections at [redacted]. I have confirmed with them that they have not responded to their statements either and the last communication they had with them was on February 20, 2013. Mrs. [redacted] states that she contacted our office and spoke with the new manager Emily.   The manager of our Richmond office is Regina and has been for over 20 years.  Mrs. [redacted] did speak with Emily but unfortunately she had contacted the New Castle Dr. Tavel office who told her that since the account was over a year old that it was beyond the insurance filing limit of one year and there was nothing that could be done and they would be responsible for the amount owed.  Mrs. [redacted] then contacted our customer service rep who explained the same thing to her.
It has now been 4 years since the services have been rendered and the patient has had his glasses.   The account remains in collection at this time with [redacted] and the amount is still owed.  Mr. [redacted] did sign the terms of sale on the patient registration form that also states that they are responsible for any charges not covered by the insurance. Had they responded to our letter that Mrs. [redacted] told our customer service rep Andrea she had received them but thought the insurance paid on the account we would not have turned the account over to collectiions and were willing to make payment arrangements per our letter.
Dr. Tavel's pursued every option to obtain the information needed to bill the primary insurance per [redacted].  Without the information [redacted] kept denying the claim so it became the responsible parties for the balance owed.  We also only billed them for what we would have received from the insurance company had we been given the information to bill the primary insurance.  Mr. [redacted] does need to contact [redacted] Services at 317-844-5103 to make payment arrangements. If you have any further questions, please contact me at 317-924-1300 X 100.   thank you,   sincerely  Ellie T[redacted]/;Director of Operations

I am responding to a complaint filed with your office on January 28,, 2016. Mr. [redacted] was examined and fit with glasses at our West 86th street Dr. Tavql office on January 6, 2016. A copy of his prescription was given to him at the time of the examination. Ihe pupillary distance measurements that...

he requested is not part of the eye examination and it Is the responsibility of the dispensing optician to take this measurement each time at the place of where the patient is purchasing the glasses. This was explained to him when he called requesting the informat on.I have enclosed a copy of Mr. [redacted] prescription which now states what his Pupillary Distance is for both far and near for the measurement that the dispensing optician measured for his glasses with Or. Tavel office.
I have tried calling Mr. [redacted] several times and have left voicemails but have not had a response as of February 5, 2016. I am mailing him a copy of the prescription attached with the information he had requested although the measurement is to be taken by the place of business that he is purchasing his additional glasses from.
I hope this answers any questions you may hovq concerning this complaint, if I may me of further assistance, please contact me at [redacted]

This is in response to a complaint that was filed with your office.  Ms. [redacted] was seen in our Terre Haute office beginning August 21, 2014 and as of today she is correct in stating that she has not received her glasses.   I have spoke with the VSP lab and they have guaranteed me that...

her glasses will be at the Terre Haute office no later that Saturday, October 25th. For the inconvienence that we have caused her with the delay of her glasses we have offered her a pair of free polarized sun glasses to wear over her contacts that she also purchased from us also.  I have also stated to Ms. [redacted] that if the glasses for any reason are not in the Terre Haute office on Saturday, October 25th then I will issue a refund in the amount of $212.00 which is what the price of the glasses are.  Ms. [redacted] has agreed to these conditions.   If I may be of further assistance please contact me at 317-667-0349 or 317-924-1300 X [redacted].

[redacted] was seen in our Zionsville office on 11-14-15 for an eye examination and glasses.  Her Daughter [redacted] did phone the next day and cancel the second pair of glasses.  The total paid was $349.98 of which $299.98 was for the second pair. She was to be refunded $299.98....

I apologize for the inconvenience we have caused in the delay of the refund.   The amount of $299.98 was credited to her card as of today, January 14, 2015.   Please allow 24 to 48 hours to appear on her bank card depending on who she banks with. If you have any further questions, please contact me at [redacted]

The salespeople at the 96th St store are rude, especially if they believe you are a medicaid customer, which I am not, but they thought I was until they looked up my info. Very, very rude and condescending were 2 of the salespeople and when I shook my head as I walked out, both rude salespeople yelled at me to "have a NICE evening!" but in the nastiest tone they both could. This all took place while another customer was present in the store. Will never do business there again.

In my opinion, the people that worked with me on getting my eyeglasses were nothing more than car salesmen. They tacked on everything possible to get the bill as high as they could. I had to tell them the things I didn't need, one of them being the protective coating, among a couple of others. I also had to remind them that the expensive pair that I was to purchase was buy 1 get 1 free. They conveniently forgot that. To this day, just over 1 year of having my glasses, the lens screw keeps working itself loose, so I have to tighten it at least once a week to keep from having the lens fall out. After the second time tightening it I discovered both lens screws were totally different. One flat head and the other Phillips head. When I confronted the doctor about it his response was that they sometimes come from the factory like that. I then told him that I had been wearing glasses for 51 years and never had I seen that before. All of this even when buy 1 get 1 free, the total bill came to $476.63. By the way, I can only rate the associates at the office at 982 N.Mitthoeffer Road. I would never recommend anybody getting eyeglasses from this office!!!
