[redacted], service date on the 14th day of September 2015. Unfortunately, Dr. Yolanda K. Vaughan passed away on the 26th day of March, 2016. We have enclosed a copy of Dr. Yolanda K. Vaughan's Death Certificate with this letter.
We have been well-informed of Ms. [redacted]'s situation concerning the billing code problem and the release of her medical chart. Review: On August 14, 2013, I received a call from Dr. Vaughns office regarding an outstanding bill that was owed to her for $143. I was told that [redacted] applied that amount to my deductible and that I owed her office the $143. Of course I disputed owing this as I knew that I do not have to pay for my well womans visits. Dr. Vaughns office told me to contact [redacted]. I asked several times what did they bill the claim for and the clerk informed me that it was for my annual visit. When I contacted [redacted], Representative [redacted], assisted me and informed that Dr. Vaughns office billed for an annual visit and a second billing came in for menstrual problems consultation. As you can imagine, I was shocked as the second visit had nothing to do with menstrual problems and I was under the impression that the second visit was a continuing visit of my well woman check up because I was there to discuss the abnormality of my pap smear results. I asked your representative if she would mind speaking with Dr. Vaughns office concerning this issue. Once I got Dr. Vaughns office on the line, the conversation went from bad to worse. Dr. Vaughn accused me and your representative of trying to force her to change her billing code. She even asked your representative to explain to me the deductible process by the way, I completely understand this process as Ive been a high option member for quite some time. I tried to explain as well as your representative that this was not the issue, the issue was that they billed for something that did not necessarily occur as they billed it. Dr. Vaughn stated that in her records she billed for weight loss consultation, which of course is not what was billed and it was not actually what took place as a result of the need to have a second visit. Dr. Vaughns statement alone raises a red flag as to what was her true intention in billing on March 21, 2013. While at the time, I was not trying to accuse her of deceptive billing, the statement that she made to me and your representative provides proof that she intended to deceive and defraud my insurance carrier. She has threatened to send me to collections if I dont pay the $143 that is owed to her. Ive never experienced such hostile treatment from a doctor that Ive entrusted to take care of my health. I am appalled and ashamed that she behaved that way to me and your representative. She was yelling and combative and refused to listen to the issue at hand as she was focused on her point of view.Desired Settlement: Refund the $143 that I have to pay in order to not be sent to collections. In addition, resubmit a corrected claim to my carrier so that her office can be paid for services rendered.
