This above issue is from July 2016, almost one year ago. Ms. [redacted] on July 22, 2016 chose and received a new pair of glasses with the same frame (size and color) as she received in 2014 and 2015, from the selection of frames allowed by her insurance company. On August 7, 2016 she was unhappy with...

her new frame and was told to call her insurance company and get authorization for a different frame. We received the authorization on August 9, 2016 and tried to call Ms. [redacted] to come in and pick out a different frame. There was no response to the phone call. On August 12, 2016 we sent a postcard to the address on file ([redacted] and still no response. As of July 22, 2017 she will be eligible for a new and different par of glasses (without prior authorization) through her insurance company, it will be one year since her last pair of glasses
