Complaint: [redacted] I am rejecting this response because: What they say doesn't even make sense, they claim I had a (pre filled out) form they read and signed and was faxed straight to my unit. However next they claim I changed the dates which is impossible. It went straight to the unit, I had a severe ankle spraying level 3 and t took a min of 3 months. He told me three months. Not once was three weeks discussed. They are making stuff up to try to justify their random dismissal. I am currently going through the process of a lawsuit if they do not right the wrong they did. I had the doctor sign maybe 5 waivers in my life time because a problem that was getting worse and worse and he was willing to put what I asked for. I did not prefill anything out.  He did so based on his findings and the severity of the injury. I had been using waivers over 11 years based on injuries without complaint. They got tired of having to fill them out and dismissed me instead of trying to figure out what was really wrong. It took me having to find a new doctor due to being rudely dismissed after 30 years to find out that I have two very bad ankles. They are so loose that I am easily spraining them by doing normal activities like walking and twisting. They would not refer me to a specialist saying it would heal but I was getting continuous sprangs which finally my new doctor referred me to a specialist who says I need ankle surgery in both ankles and it will not heal. He claims this should have been pointed out to me years ago. I can get a sparing by pivoting, having sex, walking and daily things like that now because it was ignored for so long. Now I will likely be on waivers years until both surgeries are done. They failed to do the correct action to heal my ankles and then dismissed me because they knew they jacked my ankles up and the waivers were too much for them. I only had to have a waiver filled out once every 6 months. It's impossible to change the time off, he filled it out I never changed his written dates. I will push forward with the lawsuit if they don't make this right and acknowledge they ignored a growing problem which can be seen on paper that it was getting worse. They did the same old treatment and never thought about other solutions. Like surgery you can't put a band aide of a required surgery like they kept doing for years. Plus I never once got a letter sent from them, I actually called 4 times after being dismissed to simply ask for the letter explaining why I was released to be sent and they claimed it had. Never did it come or was it sent accord to a written document from the post office. I also will be having ATT release the call recordings into court if this has to go that way. I was wronged. Period. 30 years no issues until they had to fill out simple paperwork.  Sincerely, [redacted]

majority of the time he saw Dr. [redacted].  There were a few visits that he saw Dr. [redacted]’s physician assistant.  He was seen for many things such as sore throats, sinus infections, sprains and strains, gastritis, anxiety, and other medical issues related to family practice medicine.   In May of 2015 the patient began presenting with disability forms to be filled out for excessive time off.  The forms would either remove him from the physical activity, or limit what he could physically do.  This was never a problem until his visit on March 30, 2017.  On that visit he was diagnosed with lumbar sacral strain and was told no sit ups or strenuous activity for 3 weeks.  He brought in a disability form for the doctor to fill out and pre-populated his disability dates.  The doctor approved three weeks off and the patient populated three months off. This was discovered when the employer/USAF called our office to verify the dates.  We corrected the form and told them the patient was approved for three weeks and not three months.  The patient had filled in his own dates.  Because of this action he was dismissed from our care.    A certified letter was mailed to the patient on May 17, 2017, stating he had been dismissed from our care.  The letter was unclaimed by the patient and it was returned to our office on May 19, 2017, by the postal service. He also called our office numerous times arguing over his dismissal.   We do not like to dismiss patients.  We are here to help them with all of their medical needs.  He was a patient here for a long time and dismissing patients is a hard decision.  We just could not tolerate the alterations he made to a legal form and do not wish to treat him again.   Sincerely,   Dycus Camp Clinic
