see some red flagsOur office has discounted the total fee times$discount from the $feeThe agreed fee was now $Then on another visit, the patient’s mother complained that the fee should have really been $2000, so we made another adjustment of $Now the total fee agreed was $Finally, again for an entire months worth of treatment at only $materials fee which patient has not yet finished paying forI believe our notes in the patient chart and the account history shows that we have been very accommodating to this patient and his familyWe heavily discounted the initial fee and allowed them to pay monthly for the initial placementLater for the second placement, we only charged them for the retainer and lab costs and allowed them to not make a single payment while in treatment for over a yearI have several issues with the complaintOne, the patient is complaining about the an entire new treatment plan done by an entirely new doctorHe is asking for money already paid to Dr [redacted] for a service that was done well and that patient did not have a problem until he lost his retainerThe initial treatment was completed on 9/18/which the patient never reported any dissatisfaction withThe patient ok’d the removal and accepted his retainer on 9/18/The patient only reported dissatisfaction after losing his retainer and noticing tooth movementDr [redacted] clearly warns patient and mother that a new retainer must be made immediately which patient does not get until almost months after reporting itOur consent forms clearly state that a lost or broken retainer can cause “relapse”, meaning the teeth may move back to their original state if a new retainer is not made right awayAt this point the patient is responsible for coming in and having a new retainer madeThis is not a problem caused by any of the doctorsThe second time braces were placed, they were done to keep the patient happyThey were not charged another $The first time it was done, it was done correctly and to the patient’s likingAgain, he ok’d the removal and received his retainerOver the last months this patient is still not satisfied with my workIn those months, this patient has documented missed appointments (please see account history reportDr [redacted] documented broken brackets on almost every appointment, meaning the patient was not compliant and not being careful to keep brackets in placeBroken brackets occur when patient eats hard or crunchy items and break brackets offMissed appointments and broken brackets lengthen treatmentThe patient has complained about having his teeth drilled on or adjusted, but my notes clearly state the need to “reprox” teeth to make room for tooth movementMy notes indicated no objections to this idea when my treatment plan was discussedMy current work on this patient is free of charge and if he is not happy with the work, he can leaveThe simple fact is, this patient lost his retainer, didn’t come in until about months, even after being advised by Dr [redacted] that his teeth would moveThe $(the $really) was the fee for the original treatment that was complete correctly and to his satisfaction by Dr [redacted] Resolution: The only thing Mr Belloso has paid for that he hasn’t received is his final retainerThe replacement retainer fee was charged to his account on 11/16/for $He has a $balance on his account for the lab fee, so he is owed $I am willing to refund this amount, but he will need to finish his orthodontic treatment elsewhereIf the patient wants and is willing to sign a release of liability I will remove the wires and brackets and make him his retainer at no charge and refund his $What he is asking is for $for treatment already done and completedHis complaint is that we advertised month smiles, but my notes indicate length of treatment to be months and consent signed was for traditional bracesIf I could be of any help with understanding my notes, the account history or any other matter, please call me I am rejecting this response because:Dear Revdex.com, I have several areas of concern that have raised red flags, in regards to the response sent by Dynamic DentalI'm going to address the following in order of their message: 1) "It is surprising that we should receive a complaint of any sort, and especially since this is the first time that this patient has expressed any concern or dissatisfaction." I began treatment from 11/18/- 9/18/when I only recently turned years old, as a minor who was pressured into being intimidated by this dental practice duo, naturally for the first few months I was very quiet and had a strong sense of faith with Dr***, who during this time was handling my first treatmentAfter the initial three months, it became an undeniable fact that my area of concerns weren't improving in the slightest, the following area of concerns were: my two upper teeth (which after a four year period has not been corrected) and my bottom teeth that weren't aligned properlyDuring each visit, I was told by Dr*** (as well, Dr***) that my teeth were "coming together well" and "looking very good, that there is a noticeable improvement" every time I came inThe statement mentioned above is completely false, as I've been making consistent complaints since the three month period to the dental assistants, as I would constantly express the same concern over and over again, which was: My teeth aren't visually improving, when will my bottom teeth and two front teeth be aligned?" Yes, I do admit that it took a few months for me to begin addressing my concerns, granted at my age and being assured that, "I was in good hands" with Dr***, I believed them for the time beingThe (many) previously employed dental assistants at the time have since left Dynamic dental, whether they chose to voluntarily end their employment or their employment was terminated by the Husband-Wife duoDespite mentioning my concerns with every appointment, I was assured by the dental assistants that I would be "Very content, satisfied" after the six month period, they also told me that they would let the Doctor know that I wasn't completely satisfied with the resultsDespite a young age, it became very clear to me that Dr*** wasn't able to successfully deliver a satisfied result within the six month periodI believe Dr*** (as well, Dr***) had taken advantage of the fact that I was a young adolescent who believed their every wordAt this point, I began to directly address the same area of concerns (since the start of my services with Dynamic Dental) with Dr***, who assured me that the procedure would "take a little longer" and that "the end result would have me completely satisfied." The fact that I've been addressing my concerns to the dental assistants and Dr*** during the 'guaranteed' six month service period speaks for itself, their claim that it's the first time they're received a "complaint of any sort" is completely fabricated to misdirect the attention of their consistent faults, as well the undeniable fact that during both the first and second time of treatment, they couldn't meet the month threshold that they assured me and my motherAfter the month period to complete my first treatment, I was not completely satisfied and specifically told Dr*** that I did not feel satisfied with my upper teeth, as well that their could be improvement on my bottom teeth, both of which were not properly aligned even after an month period, that really should haven't taken more than six months (if not months, granting them the benefit of the doubt.) Despite directly telling Dr***, she completely assured me that she could see the results and that there's nothing more left to improveI wasn't satisfied with her response, as well did not feel as if they were listening to any of my concerns, and with that they began impressions for my first set of retainersAt this time I felt completely dissatisfied, disregarded as a patient and mistreated, as well taken advantage because of my age - because of this circumstance was never taken seriously throughout my constant areas of concerned that I addressedThe proof is clear in the photos they attached, the result of an almost (double the assured time) of my six month smile treatment is dissatisfaction, as well misleading statements that there had been any improvement when my areas of concern weren't resolved in the slightestThis is a very foolhardy claim on their part, with all due respect, I find it unbelievable (as well, highly offensive) that they would even try to claim that I didn't make a single complaint until a year period passing, when the service was meant to deliver satisfactory results during a six month periodIt's clear to me and my mother that the dental duo is desperately trying to avoid (cover up, really) admitting any responsibility for their consistent errors, as well the indisputable fact that even after four years of initial treatment, they were both unable to deliver the bare minimum of a satisfactory result for their patientThe proof is not only in their attached photos, but the repugnant (current) condition that they've left my teeth in after recklessly trimming (as well, drilling) my teeth since the start of my treatment in with Dr*** I wholeheartedly believe that the two have been guessing and checking with my teeth, as during my treatment with Dr*** the 2nd time, he originally claimed that there was nothing he could to do fix my two front teethAfter months of trying to get my concern across to him, finally he assured me that he could continue to make space by drilling and trimming my teeth, appointment after appointmentAs a result my teeth aren't properly aligned, noticeably smaller and appear worse than the start of my initial treatmentAfter being taken advantage of for what felt like a never ending four year period, as well constant fear that any further trimming/drilling of my teeth would leave me without any hope of ever reaching the satisfactory results they had assured me of for four years agoDuring both the first and second treatment, I never reached the bare minimum of satisfaction with the results they've both assured me with every visit at Dynamic DentalAt this time, I am not only concerned for the well being of my teeth having been constantly trimmed/drilled, but completely embarrassed and ashamed of how both Dr*** and Dr*** have left the horrendous condition of my teeth. It's also clearly stated by Dynamic Dental that "both the patient and his mother reported tooth movement and dissatisfaction" since 11/11/12, with that being said, their first claim is completely invalid, it's clearly not the first time that I've "complained" or "expressed any concern or dissatisfaction about [my] treatment." On that note, 11/11/was one of many "complaints" that I had already consistently been addressing to Dr*** during the first treatmentI believe the business didn't carefully check what they had stated, it is clear that they've accidentally contradicted their own claim. In short, let me make this very simple and easy to follow: Initial statement by Dynamic Dental: "It is surprising that we should receive a complaint of any sort, and especially since this is the first time that this patient has expressed any concern or dissatisfaction." (In response to the time they received my 'first' complaint on 10/6/2015)2nd statement: "He reported losing his retainer 11/11/Both patient and his mother reported tooth movement and dissatisfaction"The key word being "dissatisfaction" has appeared twice, despite their claim that my mother and I have never expressed "dissatisfaction" until years after the start of treatment. 2) "He lost his retainer some time after this date and did not have a new retainer made until months after losing it." The 2nd claim is also carelessly fabricated, at an attempt to place fault on the patient (myself) in order to justify my teeth movement, following the loss of my retainer. Yes, it is true that I did lose retainer, any individual may easily face this circumstanceAs a result we contacted Dynamic Dental the same day (before the office closed) and several times they did not pick up our callsAs a result, my mother directly contacted Dr*** (the same day) on her personal phone number, she did in fact answer the call and said to "come tomorrow, take new impressions for new retainers, because teeth move quickly." As a result we came in the next day, they took the new impressions and we paid the $(cash) for new retainersDr*** said that it would take "a week or less, and that she would tell the lab to quickly deliver the retainers, because the teeth move quickly." Eight days had passed and we never received a call, I called the clinic and they never answered my mom's calls, once againFrom there, my mom called Dr*** and told her about the consistent discrepancies with trying to contact their dental clinic, to which she apologized for and explained that she "didn't understand why the front desk wouldn't answer her calls." At this time, the current receptionist were *** (*** perhaps) and *** who were at fault for not answering my mom's callsDr*** followed by saying "she'll my call my [mother] the following day to see if the retainers have arrived." And she did follow up with a call the next day, she said they were "not available, that she would call the lab to see what's going on" as well that "someone from the clinic would call when the retainers arrive." My mother never received a call, after an almost two week period in order to get new retainers, my mother went directly to the clinic and asked of the status of the retainers*** and *** were at the front desk and immediately apologized, they said, "we are sorry, we forgot to give you a call to pick up your retainers that came in two days ago." That being said, it is impossible that I went without a retainer for an (inflated) claim of "months" after I had originally lost my retainers. Once again, the business has made another very clear and indisputable mistake with their 2nd claim. It is stated that "He lost his retainer some time after" but the business did not provide a specified date, as well based on what they've reported: 9/11/- "Reported loss of retainers"9/30/- "Came in to take impressions" This is a week and five day period that they claim, took us to come into to take impressionsBecause this was a major priority to my mom and I, she made sure that we would come in immediately following the loss of my retainersIt certainly didn't take us a bizarre weeks and five days to come in to take impressions for my new retainers, at the very most it took us a two day period to come in, since the day I lost my retainers was on a SaturdayWhich means, the dental office would not open until the following two days, on Monday at that the start of the new weekThe business has not yet provided proof that the lab received impressions on September 30th, I would like to request that the Laboratory to personally provide when they received the impressions for my new retainers I'm going to make this very simple and short, once again:The business has made consistent claims, the proof is in the dates they've personally provided listed below. 9/30/- "Came in to take impressions" Although the business at this point has claimed, "After waiting almost months to replace retainers" meaning that the patient (myself) didn't pick up the retainers for "an almost three month period" 1/29/- "Patient's mother comes in with patient to pick up retainer" This cannot be possible, if you simply calculate the months and days the result is unanimously decided: If what they're stating is true, why is it that the time between the two dates mentioned above (When I came in to take impressions and when my mother comes in to pick up the retainer) a month and day periodThey already stated that it took an "almost three month period to replace the retainers" The facts are listed, there are constant errors in their claims that prove what the business is stating is not true at allAt this point, we can continue to expose the errors and faults (really, the lies) Dr***, as well Dr*** are trying to conjure, in order to cover up their consecutive mistakesAll of the evidence proves that they are blatantly lying, throughout their response to my complaint. On another note, they claimed that I was with my mother during the day that we picked up the retainers, which is just another example of how inaccurate their claims areI was not with my mom at the time, as I was at school and she went to pick it up with her companion, a female. In regards to the closing thoughts made by Dr***, yes I have missed eight appointments due a tight school scheduleYes, it is true that my brackets (only) during the first treatment fell off, although I wholeheartedly disagree and know for a fact that it wasn't "on almost every appointment" as the business claimedI honestly don't even have to prove this, as they've already exaggerated a lot of statements and made blatant lies as listed aboveAs for broken (one or two occasional) brackets, this is not only completely but to be accepted for anyone who has bracesDr*** and Dr*** are both completely aware that I am a responsible, meticulous and careful young man - I find this very hypocritical that they would state that I'm not, "compliant and not being careful to keep brackets in place." I personally ate very carefully (soft foods, not hard) and still managed at the time of the first treatment to have broken brackets, this is simply because the Dental assistants (who have either left Dynamic Dental or have had their employment terminated) were inexperienced, on several occasions no more than a few hours after my appointments, a bracket would fall of because of the poor delivery of placing the brackets. 3) "The patient has complained about having his teeth drilled on or adjusted, but my notes clearly state the need to "reprox" teeth to make room for tooth movementMy notes indicated no objection to this idea when my treatment plan was discussed." Although at first I agreed because Dr*** said that my teeth would finally be a satisfactory result for me, and I allowed him to drill and adjust my teeth. However, this is not completely accurate, as after a fifth time that he began to drill and trim my teeth, I continued to ask him specifically if this was completely necessaryI noted that it was fairly painful because my teeth are very sensitive, and as a result of consistent drilling and trimming, they've become even more sensitive than beforeAt which point, I once again addressed that this was an arising issue, to which he replied, "In order to get your desired results and be satisfied, you would need to continue allowing me to drill and trim your teeth accordingly." Dr*** did not offer any alternative, as he continued to assure me with every appointment that my teeth was "coming together very nicely." Not only was this evident, Dr*** continued to push me during the last three months of my 2nd treatment to remove my braces and begin my impressionsAt my most recent appointment in Dynamic Dental (9/29/2015) I arrived only to be told by Dr*** and the dental assistants that I would conclude with my impressions that day, ordered by Dr***I told the dental assistant (I do not know her name, I know (based off what she's told me) that she is currently attending Sacramento City CollegeThat being said, that day (for some very unusual reason that was never explained to me) a new dental assistant was working on removing my wires that day, she is hispanic herselfAlthough she switched out with the other dental assistant because she was still very inexperienced, the one who's attending Sacramento City College removed them and had Dr*** check my teethAs always, Dr*** stated that my teeth appeared "Very nice" and that I was "more than ready for my impressions." Dr*** left the room, and I promptly told the dental assistant that I was not happy or nearly satisfied with the results, and that there was no way that I would agree to take impressions on 9/29/As a result, they had already taken out the wire (refer to the attached photos listed in this message), because I have an iPhone the date and time is displayed as wellDr*** attempted to convince me that my teeth were straight, after I continued to tell him that they were no where near ready for impressions, and that I did not appreciate him never discussing impressions prior to 9/29/After a short while, Dr*** did not continue to try and convince me, and began to (once again) drill and trim my teeth until it began to bleed, the dental assistant gave me a mirror to check tooI floss and brush everyday, so I believe the result of my gum bleeding was because the pressure he placed onto my teethThe dental assistant then put on my wire back (refer to my attached photo, as I still have my braces intact, as of this moment.) Conclusion: My name is *** *** and I am currently nineteen years old, my request directly to Dr*** and Dr*** (Dynamic Dental) is to take full responsibility for the inconvenience they've brought to me over a four year periodI want to go see an Orthodontic specialist of my choosing, and I would like to see what is the current condition of my teethI am demanding that Dr*** and Dr*** pay for my initial visit, as well any treatment that I require for the new Orthodontist specialist in order to fulfill the satisfaction that Dynamic Dental failed to meet in a four year period, under a six month program. Whether this is a total of $or more, both Dr*** and Dr*** must pay for my new treatments accordingly *** ***

Our office has discounted the total fee 3 times. $1500 discount from the $4000 normal fee. The agreed fee was now $2500. Then on another visit, the patient’s mother complained that the fee should have really been $2000, so we made another adjustment of $540. Now the total fee agreed was $2000. Finally, again for an entire 24 months worth of treatment at only $408 materials fee which patient has not yet finished paying for. I believe our notes in the patient chart and the account history shows that we have been very accommodating to this patient and his family. We heavily discounted the initial fee and allowed them to pay monthly for the initial placement. Later for the second placement, we only charged them for the retainer and lab costs and allowed them to not make a single payment while in treatment for over a year. I have several issues with the complaint. One, the patient is complaining about the an entire new treatment plan done by an entirely new doctor. He is asking for money already paid to Dr [redacted] for a service that was done well and that patient did not have a problem until he lost his retainer. The initial treatment was completed on 9/18/2012 which the patient never reported any dissatisfaction with. The patient ok’d the removal and accepted his retainer on 9/18/2012. The patient only reported dissatisfaction after losing his retainer and noticing tooth movement. Dr [redacted] clearly warns patient and mother that a new retainer must be made immediately which patient does not get until almost 3 months after reporting it. Our consent forms clearly state that a lost or broken retainer can cause “relapse”, meaning the teeth may move back to their original state if a new retainer is not made right away. At this point the patient is responsible for coming in and having a new retainer made. This is not a problem caused by any of the doctors. The second time braces were placed, they were done to keep the patient happy. They were not charged another $2500. The first time it was done, it was done correctly and to the patient’s liking. Again, he ok’d the removal and received his retainer. Over the last 24 months this patient is still not satisfied with my work. In those 24 months, this patient has 8 documented missed appointments (please see account history report. Dr [redacted] documented broken brackets on almost every appointment, meaning the patient was not compliant and not being careful to keep brackets in place. Broken brackets occur when patient eats hard or crunchy items and break brackets off. Missed appointments and broken brackets lengthen treatment. The patient has complained about having his teeth drilled on or adjusted, but my notes clearly state the need to “reprox” teeth to make room for tooth movement. My notes indicated no objections to this idea when my treatment plan was discussed. My current work on this patient is free of charge and if he is not happy with the work, he can leave. The simple fact is, this patient lost his retainer, didn’t come in until about 3 months, even after being advised by Dr. [redacted] that his teeth would move. The $2500 (the $2000 really) was the fee for the original treatment that was complete correctly and to his satisfaction by Dr [redacted]. Resolution: The only thing Mr Belloso has paid for that he hasn’t received is his final retainer. The replacement retainer fee was charged to his account on 11/16/2012 for $300. He has a $53 balance on his account for the lab fee, so he is owed $247. I am willing to refund this amount, but he will need to finish his orthodontic treatment elsewhere. If the patient wants and is willing to sign a release of liability I will remove the wires and brackets and make him his retainer at no charge and refund his $247. What he is asking is for $2500 for treatment already done and completed. His complaint is that we advertised 6 month smiles, but my notes indicate length of treatment to be 9 months and consent signed was for traditional braces. Review: I first emailed the dentist to request copies of full mouth x-rays for my family, as we were going to a new dentist. I got a return email to call the office. I called and was told that the x-rays would cost $35.00 per person. I then mailed a letter to them on 9/**/13, stating I only needed copies of FULL MOUTH X_RAYS sent for my husband and younger daughter, with a check for $70.00. When I arrived for my appointment with new dentist I was told that Bitewing x-rays were sent, not the full mouth as requested. They could not use them, they gave them to me to take home. They had to take new xrays.

They told me if I wanted full mouth xrays it would take a while to get. I should have been told that when I spoke to them on the phone initially. I have tried several times to request a refund of my check. My last correspondence to them they never even answered.Desired Settlement: I want a refund check in the amount of $70.00.

A request for duplication of dental radiographs was received by the complainant for two family members. As a courtesy, and in an attempt to expedite the delivery to the new dental office, the most recent X-rays were sent via mail.

A payment of $70.00 was received and the X-rays were delivered as agreed upon.

When the complainant contacted us again to state that the X-rays were received but not used and that she needed the old full mouth series of X-rays from 2011, we told her that we would be happy to do so without any further charge. The option of having them mailed to her, to the new office or for her to directly pick them up was given.

The complainant replied that she already had her family members take new full mouth X-rays at the current dental office. Despite this option that was given the patients chose to have X-rays taken elsewhere. If it was essential for the 2011 radiographs to be utilized, the patients had the option of waiting for their duplication. Instead they decided to take new X-rays.

Because the X-rays were duplicated, processed and sent to the office of the patients' choosing, services were rendered and no refund is due.

If the complainant still requires those additional X-rays, we will further extend a courtesy and have them sent to the current office.

Documentation: taken from computer patient chart (notes) and Account History Report, both areas document any financial transactions and clinical procedures performed on patients. Print­outs are included. Brief Account of Case: Patient has had orthodontic (braces) treatment done twice. Once by Dr [redacted] and once by Dr [redacted]. Once by Dr [redacted] for $2000 from 11/18/2011 through 9/18/2012 to patient’s satisfaction. He ok’d the removal of braces and accepted his retainer. He lost his retainer some time after this date and did not have a new retainer made until 3 months after losing it. He reported losing his retainer on 11/11/12. He never reported any dissatisfaction after Dr [redacted] removed braces. Both patient and his mother reported tooth movement and dissatisfaction with tooth movement after losing retainer. Dr [redacted]’s notes clearly states that patient needed to come in ASAP to have a new retainer made to prevent any further tooth movement. Patient did not come in for a new retainer until 3 months after reporting tooth movement. Doctors felt bad for him and wanted to keep patient happy and agreed to treat patient a second time at no charge. Patient opted to have a removable retainer made, and not have a bonded retainer placed. 11/11/12: Patient reported losing removable retainer. Dr. [redacted] wrote in her chart that she advised patient “to come ASAP to have (new retainers) made if not teeth would shift.” 11/30/12: Patient comes in to have impressions taken for new retainers. I am rejecting this response because:Dear Revdex.com, I have several areas of concern that have raised red flags, in regards to the response sent by Dynamic Dental. I'm going to address the following in order of their message: 1) "It is surprising that we should receive a complaint of any sort, and especially since this is the first time that this patient has expressed any concern or dissatisfaction." I began treatment from 11/18/2011 - 9/18/2012 when I only recently turned 15 years old, as a minor who was pressured into being intimidated by this dental practice duo, naturally for the first few months I was very quiet and had a strong sense of faith with Dr. [redacted], who during this time was handling my first treatment. After the initial three months, it became an undeniable fact that my area of concerns weren't improving in the slightest, the following area of concerns were: my two upper teeth (which after a four year period has not been corrected) and my bottom teeth that weren't aligned properly. During each visit, I was told by Dr. [redacted] (as well, Dr. [redacted]) that my teeth were "coming together well" and "looking very good, that there is a noticeable improvement" every time I came in. The statement mentioned above is completely false, as I've been making consistent complaints since the three month period to the dental assistants, as I would constantly express the same concern over and over again, which was: My teeth aren't visually improving, when will my bottom teeth and two front teeth be aligned?" Yes, I do admit that it took a few months for me to begin addressing my concerns, granted at my age and being assured that, "I was in good hands" with Dr. [redacted], I believed them for the time being. The (many) previously employed dental assistants at the time have since left Dynamic dental, whether they chose to voluntarily end their employment or their employment was terminated by the Husband-Wife duo. Despite mentioning my concerns with every appointment, I was assured by the dental assistants that I would be "Very content, satisfied" after the six month period, they also told me that they would let the Doctor know that I wasn't completely satisfied with the results. Despite a young age, it became very clear to me that Dr. [redacted] wasn't able to successfully deliver a satisfied result within the six month period. I believe Dr. [redacted] (as well, Dr. [redacted]) had taken advantage of the fact that I was a young adolescent who believed their every word. At this point, I began to directly address the same area of concerns (since the start of my services with Dynamic Dental) with Dr. [redacted], who assured me that the procedure would "take a little longer" and that "the end result would have me completely satisfied." The fact that I've been addressing my concerns to the dental assistants and Dr. [redacted] during the 'guaranteed' six month service period speaks for itself, their claim that it's the first time they're received a "complaint of any sort" is completely fabricated to misdirect the attention of their consistent faults, as well the undeniable fact that during both the first and second time of treatment, they couldn't meet the 6 month threshold that they assured me and my mother. After the 11 month period to complete my first treatment, I was not completely satisfied and specifically told Dr. [redacted] that I did not feel satisfied with my upper teeth, as well that their could be improvement on my bottom teeth, both of which were not properly aligned even after an 11 month period, that really should haven't taken more than six months (if not 8 months, granting them the benefit of the doubt.) Despite directly telling Dr. [redacted], she completely assured me that she could see the results and that there's nothing more left to improve. I wasn't satisfied with her response, as well did not feel as if they were listening to any of my concerns, and with that they began impressions for my first set of retainers. At this time I felt completely dissatisfied, disregarded as a patient and mistreated, as well taken advantage because of my age - because of this circumstance was never taken seriously throughout my constant areas of concerned that I addressed. The proof is clear in the photos they attached, the result of an almost (double the assured time) of my six month smile treatment is dissatisfaction, as well misleading statements that there had been any improvement when my areas of concern weren't resolved in the slightest. This is a very foolhardy claim on their part, with all due respect, I find it unbelievable (as well, highly offensive) that they would even try to claim that I didn't make a single complaint until a 4 year period passing, when the service was meant to deliver satisfactory results during a six month period. It's clear to me and my mother that the dental duo is desperately trying to avoid (cover up, really) admitting any responsibility for their consistent errors, as well the indisputable fact that even after four years of initial treatment, they were both unable to deliver the bare minimum of a satisfactory result for their patient. The proof is not only in their attached photos, but the repugnant (current) condition that they've left my teeth in after recklessly trimming (as well, drilling) my teeth since the start of my treatment in 2011 with Dr. [redacted] I wholeheartedly believe that the two have been guessing and checking with my teeth, as during my treatment with Dr. [redacted] the 2nd time, he originally claimed that there was nothing he could to do fix my two front teeth. After months of trying to get my concern across to him, finally he assured me that he could continue to make space by drilling and trimming my teeth, appointment after appointment. As a result my teeth aren't properly aligned, noticeably smaller and appear worse than the start of my initial treatment. After being taken advantage of for what felt like a never ending four year period, as well constant fear that any further trimming/drilling of my teeth would leave me without any hope of ever reaching the satisfactory results they had assured me of for four years ago. During both the first and second treatment, I never reached the bare minimum of satisfaction with the results they've both assured me with every visit at Dynamic Dental. At this time, I am not only concerned for the well being of my teeth having been constantly trimmed/drilled, but completely embarrassed and ashamed of how both Dr. [redacted] and Dr. [redacted] have left the horrendous condition of my teeth. It's also clearly stated by Dynamic Dental that "both the patient and his mother reported tooth movement and dissatisfaction" since 11/11/12, with that being said, their first claim is completely invalid, it's clearly not the first time that I've "complained" or "expressed any concern or dissatisfaction about [my] treatment." On that note, 11/11/12 was one of many "complaints" that I had already consistently been addressing to Dr. [redacted] during the first treatment. I believe the business didn't carefully check what they had stated, it is clear that they've accidentally contradicted their own claim. In short, let me make this very simple and easy to follow: Initial statement by Dynamic Dental: "It is surprising that we should receive a complaint of any sort, and especially since this is the first time that this patient has expressed any concern or dissatisfaction." (In response to the time they received my 'first' complaint on 10/6/2015)2nd statement: "He reported losing his retainer 11/11/12. Both patient and his mother reported tooth movement and dissatisfaction"The key word being "dissatisfaction" has appeared twice, despite their claim that my mother and I have never expressed "dissatisfaction" until 4 years after the start of treatment. 2) "He lost his retainer some time after this date and did not have a new retainer made until 3 months after losing it." The 2nd claim is also carelessly fabricated, at an attempt to place fault on the patient (myself) in order to justify my teeth movement, following the loss of my retainer. Yes, it is true that I did lose retainer, any individual may easily face this circumstance. As a result we contacted Dynamic Dental the same day (before the office closed) and several times they did not pick up our calls. As a result, my mother directly contacted Dr. [redacted] (the same day) on her personal phone number, she did in fact answer the call and said to "come tomorrow, take new impressions for new retainers, because teeth move quickly." As a result we came in the next day, they took the new impressions and we paid the $300 (cash) for new retainers. Dr. [redacted] said that it would take "a week or less, and that she would tell the lab to quickly deliver the retainers, because the teeth move quickly." Eight days had passed and we never received a call, I called the clinic and they never answered my mom's calls, once again. From there, my mom called Dr. [redacted] and told her about the consistent discrepancies with trying to contact their dental clinic, to which she apologized for and explained that she "didn't understand why the front desk wouldn't answer her calls." At this time, the current receptionist were [redacted] ([redacted] perhaps) and [redacted] who were at fault for not answering my mom's calls. Dr. [redacted] followed by saying "she'll my call my [mother] the following day to see if the retainers have arrived." And she did follow up with a call the next day, she said they were "not available, that she would call the lab to see what's going on" as well that "someone from the clinic would call when the retainers arrive." My mother never received a call, after an almost two week period in order to get new retainers, my mother went directly to the clinic and asked of the status of the retainers. [redacted] and [redacted] were at the front desk and immediately apologized, they said, "we are sorry, we forgot to give you a call to pick up your retainers that came in two days ago." That being said, it is impossible that I went without a retainer for an (inflated) claim of "3 months" after I had originally lost my retainers. Once again, the business has made another very clear and indisputable mistake with their 2nd claim. It is stated that "He lost his retainer some time after" but the business did not provide a specified date, as well based on what they've reported: 9/11/2012 - "Reported loss of retainers"9/30/2012 - "Came in to take impressions" This is a 2 week and five day period that they claim, took us to come into to take impressions. Because this was a major priority to my mom and I, she made sure that we would come in immediately following the loss of my retainers. It certainly didn't take us a bizarre 2 weeks and five days to come in to take impressions for my new retainers, at the very most it took us a two day period to come in, since the day I lost my retainers was on a Saturday. Which means, the dental office would not open until the following two days, on Monday at that the start of the new week. The business has not yet provided proof that the lab received impressions on September 30th, 2012. I would like to request that the Laboratory to personally provide when they received the impressions for my new retainers. I'm going to make this very simple and short, once again:The business has made consistent false claims, the proof is in the dates they've personally provided listed below. 9/30/2012 - "Came in to take impressions" Although the business at this point has claimed, "After waiting almost 3 months to replace retainers" meaning that the patient (myself) didn't pick up the retainers for "an almost three month period" 1/29/2013 - "Patient's mother comes in with patient to pick up retainer" This cannot be possible, if you simply calculate the months and days the result is unanimously decided: If what they're stating is true, why is it that the time between the two dates mentioned above (When I came in to take impressions and when my mother comes in to pick up the retainer) a 3 month and 29 day period. They already stated that it took an "almost three month period to replace the retainers" The facts are listed, there are constant errors in their claims that prove what the business is stating is not true at all. At this point, we can continue to expose the errors and faults (really, the lies) Dr. [redacted], as well Dr. [redacted] are trying to conjure, in order to cover up their consecutive mistakes. All of the evidence proves that they are blatantly lying, throughout their response to my complaint. On another note, they claimed that I was with my mother during the day that we picked up the retainers, which is just another example of how inaccurate their claims are. I was not with my mom at the time, as I was at school and she went to pick it up with her companion, a female. In regards to the closing thoughts made by Dr. [redacted], yes I have missed eight appointments due a tight school schedule. Yes, it is true that my brackets (only) during the first treatment fell off, although I wholeheartedly disagree and know for a fact that it wasn't "on almost every appointment" as the business claimed. I honestly don't even have to prove this, as they've already exaggerated a lot of statements and made blatant lies as listed above. As for broken (one or two occasional) brackets, this is not only completely normal but to be accepted for anyone who has braces. Dr. [redacted] and Dr. [redacted] are both completely aware that I am a responsible, meticulous and careful young man - I find this very hypocritical that they would state that I'm not, "compliant and not being careful to keep brackets in place." I personally ate very carefully (soft foods, not hard) and still managed at the time of the first treatment to have broken brackets, this is simply because the Dental assistants (who have either left Dynamic Dental or have had their employment terminated) were inexperienced, on several occasions no more than a few hours after my appointments, a bracket would fall of because of the poor delivery of placing the brackets. 3) "The patient has complained about having his teeth drilled on or adjusted, but my notes clearly state the need to "reprox" teeth to make room for tooth movement. My notes indicated no objection to this idea when my treatment plan was discussed." Although at first I agreed because Dr. [redacted] said that my teeth would finally be a satisfactory result for me, and I allowed him to drill and adjust my teeth. However, this is not completely accurate, as after a fifth time that he began to drill and trim my teeth, I continued to ask him specifically if this was completely necessary. I noted that it was fairly painful because my teeth are very sensitive, and as a result of consistent drilling and trimming, they've become even more sensitive than before. At which point, I once again addressed that this was an arising issue, to which he replied, "In order to get your desired results and be satisfied, you would need to continue allowing me to drill and trim your teeth accordingly." Dr. [redacted] did not offer any alternative, as he continued to assure me with every appointment that my teeth was "coming together very nicely." Not only was this evident, Dr. [redacted] continued to push me during the last three months of my 2nd treatment to remove my braces and begin my impressions. At my most recent appointment in Dynamic Dental (9/29/2015) I arrived only to be told by Dr. [redacted] and the dental assistants that I would conclude with my impressions that day, ordered by Dr. [redacted]. I told the dental assistant (I do not know her name, I know (based off what she's told me) that she is currently attending Sacramento City College. That being said, that day (for some very unusual reason that was never explained to me) a new dental assistant was working on removing my wires that day, she is hispanic herself. Although she switched out with the other dental assistant because she was still very inexperienced, the one who's attending Sacramento City College removed them and had Dr. [redacted] check my teeth. As always, Dr. [redacted] stated that my teeth appeared "Very nice" and that I was "more than ready for my impressions." Dr. [redacted] left the room, and I promptly told the dental assistant that I was not happy or nearly satisfied with the results, and that there was no way that I would agree to take impressions on 9/29/15. As a result, they had already taken out the wire (refer to the attached photos listed in this message), because I have an iPhone the date and time is displayed as well. Dr. [redacted] attempted to convince me that my teeth were straight, after I continued to tell him that they were no where near ready for impressions, and that I did not appreciate him never discussing impressions prior to 9/29/15. After a short while, Dr. [redacted] did not continue to try and convince me, and began to (once again) drill and trim my teeth until it began to bleed, the dental assistant gave me a mirror to check too. I floss and brush everyday, so I believe the result of my gum bleeding was because the pressure he placed onto my teeth. The dental assistant then put on my wire back (refer to my attached photo, as I still have my braces intact, as of this moment.) Conclusion: My name is [redacted] and I am currently nineteen years old, my request directly to Dr. [redacted] and Dr. [redacted] (Dynamic Dental) is to take full responsibility for the inconvenience they've brought to me over a four year period. I want to go see an Orthodontic specialist of my choosing, and I would like to see what is the current condition of my teeth. I am demanding that Dr. [redacted] and Dr. [redacted] pay for my initial visit, as well any treatment that I require for the new Orthodontist specialist in order to fulfill the satisfaction that Dynamic Dental failed to meet in a four year period, under a six month program. Whether this is a total of $2500 or more, both Dr. [redacted] and Dr. [redacted] must pay for my new treatments accordingly. [redacted]
