Prop. Treas. Reg.1.12506(b)(3)(i) & Prop. Treas. Reg.1.125-6(b)(2), all debit card transactions must be substantiated. Most charges are automatically substantiated at the point of sale. Examples of this would be regular co-pays, recurring expenses and most prescriptions.
When charges are not able to automatically substantiate, a letter is sent to the participant requesting a copy of an itemized receipt/bill or an Explanation of Benefits. If Eide Bailly does not receive a response from the first inquiry, a second notice is sent. An additional 14 days is given to respond to the second request. If we do not receive a response to the second request an automatic overpayment letter is sent and the debit card is placed in a suspended status. The card is made active when the requested documentation is received.
As in this case, there was a debit card transaction on 5/13/15 in the amount of $75.95. Since the transaction did not auto-substantiate, a First Receipt Request letter was mailed to the account holder, Shirley [redacted] on 7/22/15. We received documentation which was processed on 7/31/15. The documentation received did not show the date of service or the type of service received. A letter was sent on 8/1/15 requesting an itemized statement.
Mr. [redacted] called on 8/10/15 inquiring on the status of the transaction. We communicated to him that we had not received the additional documentation we had requested. Since no additional documentation was received, the debit card was put into suspended status on 8/31/15 and notification was sent to the account holder. The requested itemized documentation was received and processed on 9/3/15 and the debit card was moved to an active status.

It's should be quite obvious when the card is used to pay a medical clinic !!!
