STOP the pronoun usage
Let me start by saying I love my accountant at your firm in Norman, Oklahoma! However, I can only assume her declaration and clarification of her pronouns in her recent emails came from her corporate office. STOP ALREADY! I am ending 20 year relationships with my many vendors who do are buying into cancel culture. This is the only warning I’ll give you.
If it doesn’t stop, I’m gone. I spend thousands of dollars w Eide Bailly annually. I. Do. Not. Care. The future of this country is more important to me than a 10 year relationship with Eide Bailly. Stop
The nonsense. Grow a set.
Stop giving into the ridiculousness of a small group of people. There is no way trans and “binary” are the majority. They can do whatever they want personally, but they have gone too far with this stupidity. Stop! I give you my word- I will be gone if I continue seeing pronouns on my email exchanges.

Our partner did speak with Dr*** and his spouse after they reached out to us regarding the bill and indicated that if they didn’t see value in the services provided, they could simply ignore the bill, or pay what they thought was of valueBased on that discussion, we wrote off the entire
amount, only to be surprised later with full paymentWe are submitting a check request now (March 8, 2018) that will be mailed back to Dr*** for the same amount of his payment to us received January 12,

Initial Business Response /* (1000, 9, 2015/04/28) */
In response to your letter dated April 17, 2015 to the Revdex.com we want to apologize for your frustration with the debit card process for your flexible spending account.
Our records show that in 2015 there was a total of 1 debit...

transaction in the amount of $86.50 to [redacted] DDS on 1/16/15. The first Receipt Request was sent to you on 4/2/15. As of this date, we have not received the requested documentation.
In plan year 2013 there was 1 debit card transaction on 12/12/13 in the amount of $180.00 for which we requested and received documentation from you and the transaction was substantiated. In the plan year 2014 there was a total of 2 debit transactions. The first was on 6/25/14 in the amount of $74.64 which was declined at the point of sale for insufficient funds. The second transaction was also on 6/25/14 in the amount of $4.41 which was approved and substantiated at the point of sale.
As required by the Internal Revenue Service, each and every debit card transaction must be verified. Most transactions are verified through the automatic verification process by which carrier feeds are sent to us to reconcile debit card transactions, but some few transactions do not match. For those that do not match, even very small dollar amounts must be verified as valid transactions. The process of requesting documentation is not our rule - it is a requirement of the IRS for this program. In order to comply with IRS regulations with which the State of Minnesota's plan must also comply, we, as the State's administrator, require this information.
Because so many participants feel that the debit card makes the reimbursement process paperless, in each quarterly letter that we mail to participants, an information sheet called "Using the Benny Card" is included. A copy of that document is attached for your review.
Documentation that is faxed is received and stored electronically. It can be located if we are provided with the fax number it was faxed from as well as the approximate date and time it was faxed. Documentation that is emailed can be located with the email address from which it was sent. Additionally, participants have the option of logging into their account via our Consumer Portal through their computer or smart phone and uploading the documentation as well.
Your debit card is in an active status and according to our records the card has never been in a suspended/hold status.
Our records indicate that 2015 is the first year you enrolled in the Medical Dental Expense Account (MDEA) with an election of $2550.00. The contributions for this account are deducted on a pre-tax basis from your paycheck in equal increments for 24 pay period by the employer. In previous years you were enrolled in an HRA plan that was funded entirely by your employer, the State of Minnesota. The balance in this account rolled from one year to the next and no money was forfeited.

Using the Benny Card
What is it? The Benny Card is a debit card that allows you to pay for eligible medical, dental, or vision expenses through the tax-free Medical/Dental Expense Account (MDEA) and/or the Health Reimbursement Arrangement (HRA).I Using the card does not mean that all transactions are paperless. The Internal Revenue Service (IRS) requires that ALL debit card transactions including those made using the Benny Card be documented as eligible expenses.ii Most transactions are verified automatically. However, some will require documentation from the participant as substantiation for a specific debit card transaction.
How to use it:
o Use your Benny Card like a debit card when paying for eligible expenses.
o Use it only for a current year's expense.
o Save all itemized receipts or Explanations of Benefits (EOBs) as documentation. (See below for what is considered acceptable documentation.)
o The date the card is swiped is the date of the transaction. It may not correspond to the date service was provided.
o The card is limited by the amount in your MDEA and/or HRA.
If you receive a substantiation request from Eide Bailly:
o You must submit the documentation via fax, U.S. mail, mobile app, or online to Eide Bailly.
o If you do not provide the substantiation in a timely manner:
? Your debit card will be turned off.
? Your debit card transaction will be considered a collectible debt. If the amount is under $25, this amount will be included on your W2 form as taxable income. If the amount is $25 or greater, this amount will be reclaimed through the Department of Revenue's Revenue Recapture system. This means that if you receive a state tax refund or property tax refund, your debt will be deducted from that refund.
What is acceptable documentation?
o Acceptable documentation is an itemized receipt or Explanation of Benefits (EOB) that reflects the actual date of service, description of service and patient portion of the charges.
o Remember that the Benny Card should only be used for current year's expenses. Paying a prior year's expense or pre-paying for future services is not allowed.

Initial Consumer Rebuttal /* (3000, 11, 2015/04/28) */
(The consumer indicated he/she DID NOT accept the response from the business.)
The main problem that we have is that we do send in reciepts right away but sometimes we are sending them 2-3 times and Eide Bailly claims they are not recieved and we end up with the bill. That's why we have so few recent transactions. We need to get our family in for appointments but we are reluctant to make those appointments because the co-pays are not honored like they should be when recipts are recieved. The $86.50 to [redacted] on 1/16/15 that you claim you have not recieved documentation for was faxed both by us and the dentists office on 1/16/15 both faxes went through successfully on our end. I have attached a copy of the original recipts as well as the most recent statement from the [redacted] dated 3/26/2015.
Final Business Response /* (4000, 13, 2015/05/06) */
In regards to your statement "We do send in receipts right away but sometimes we are sending them 2-3 times", our suggestion is to wait to receive a Debit Card Receipt Request Letter before submitting documentation. By doing so you can attach a copy of the request letter to the documentation as this is the best way to identify the transaction for which you are submitting. If documentation is sent before the transaction has gone through the auto-substantiation process in our system and the receipt request letter is issued, we have no way of matching the documentation with the debit card transaction. In addition to fax or mail, you can also upload your documentation by logging into your account online via the Consumer Portal at www.eidebaillybenefits.com or using our smart phone app.
Additionally, if the card is being used for copays for the State of Minnesota health plans, those copays are loaded into our system and a debit transaction will automatically match and no documentation would be needed. Our system does not show any declined transactions or attempts to use the card for these copays.
We tried to reach you by phone to discuss this. If you have any questions please contact our customer service department at [redacted] or [redacted] between the hours of 7:30 a.m. and 5:00 p.m. Central time.
