I received a letter from Revdex.com, on Saturday, January 14, I visit Elder Creek Dental on August 04, it wasn't emergencyI was having problem with my tooth, it was little sharking, Elder Creek Dental did a x-ray and told me that "I need a have a root canal with specialist" Reception called and made my appointment with specialist I paid $in total for root canalI never complained about my any other tooth but they take x-ray of other teeth after one week later I went back to have filling on root canal toothI told reception that "i'm here to get my tooth filling which I have root canal done because any time eat food it get stuck inI don't know why they left that tooth and did filling in other teeth when I wasn't having any problemOnce again i'm telling you with my honestly and very truthfully words they make me sign the agreement and charge me $for teethWhen they told me they done then I talk to my wife they haven't did the filling on that tooth that I'm here forWe complain to dentistReception cam to us in room and told me that it will be a different charge for that tooth $to buildup and she make sign another agreementI told the reception that's why i'm here for, so they fill that too same time after they done we went to reception and the total amount she told us $which my wife paid them with her card right on spotThey ask me have crown I told them not right now may be later because I expend too much money alreadyAfter one week later I went to dentist to get one of my tooth pull and they charge me $plus $for office visitI paid them right on the spot and asked them that all my bill is paid for my portion, she told me and wife you are good, you paid all your shareAfter one month later I received a bill than I call them I paid all bills and you people told me everything paid then why sending me bills and what kind of charges is this, I talk to office manager and she started getting rough on me telling me to go find other dentist don't come back here I told her I will complain to Revdex.com which I didShe did races and harassing meAfter 2nd month I received the bill again and took the and went into their office and talk to reception at that the office was not infirst told me that she don't know what kind of charges is that, then she change the story and telling me that I told her that I will come back and pay the rest of balance, which I never said that, I paid everything in full same time.$is not a large amount but I want them do this right in first place: Please ask them to dispute the charges.Yours Truthfully [redacted] ***

Patient *** *** came into our office for an emergency visit on August 04, Normally we only look at one tooth during an emergency visit, but patient complained of pain on all four molar areas so we took x-rays of his back teethWe are contracted with his insurance plan and must charge
the patient their fee scheduleOn that day we did a treatment plan of all the work that the patient needed on the teeth we took x-rays onThis treatment included fillings, extraction, teeth that needed root canals, buildups and crownsHe was also told that he needed to get a consultation on a tooth at an endodontist to see if the tooth needed a root canal or extractionHe was given and signed a treatment plan that included all of the treatment that we can do in our office along with the treatment for the buildup and crown on the tooth that we sent him to the endodontist for if they deemed it was savableThis included the fees from the fee schedule provided to us from his insurance companyPatient was given a copy of this treatment plan that he signed.We made patient an appointment for fillings from the plan we gave him of treatment that we could do in our office for August 23, When a patient comes in for treatment they are given a consent form for the treatment that they are getting that day before they are seatedIn this case it was for fillings on teeth teethHe signed this consent and was taken to the back for the treatmentPatient had the root canal done at the specialist on August 16thPatient did not want to work in that tooth because of the cost of the crownWe explained to him that we could just do the build up in the tooth and he could have the crown done laterBecause this is a chewing tooth we recommend that he does not wait too long to get the crown for the protection that it provides for teeth that have had root canals on them as with the blood supply to the tooth removed they will get brittle over timePatient agreed to do the buildup on the tooth and wanted another quote showing the cost for the partial treatment planWe printed out a treatment plan that included the fillings, the buildup and the extraction that he needed doneThis also had the fees from his insurance fee scheduleThis was not additional treatment but a portion of his original treatment plan that showed the cost of today’s treatment and the extraction that he wanted doneThis treatment plan along with the consent form for the addition of the buildup was taken to him in the back for his signaturePatient signed the consent form and I believe he kept the copy of the treatment plane with the feesWe then did the buildup on the tooth.Patient came back into our office for an emergency visit on September 9thThis time patient had pain on an anterior toothWe did not have any x-rays of the anterior teeth as patient had not yet had a complete examWe took an x-ray of the area where the patient said the pain was coming formAn anterior tooth had a large filling with recurrent carries and doctor diagnosed a root canal, buildup and crown for that toothThis is the only change that was made to his treatment plan that he was given on his first visit to our officePatient was given a prescription for a pain killer and an antibioticPatient did not want to schedule an appointment for this treatment because of the cost so we scheduled an appointment for him to get the extraction diagnosed on his first visit for September 9th.Patient came for his visit on September 9th and signed his consent for the extraction and we extracted his tooth.Patient’s insurance requires the patient to pay a $office visit fee each time he comes to see usHe did not pay this fee on August 4thThe cost of the treatment that he had done on August 23rd was $for the fillings, $for the buildup and his $office visit feeThis brought his balance to $for the two visitsThe patient paid $on his visaThis left a balance of $Patient had a $office visit fee for his visit on September 1st but did not pay us anything toward what he owed us at that timePatient’s final visit with us on September 8th was $for his extraction and his $office visit feeHe paid $160.00, the amount for that day’s visit but nothing toward the past balance on the accountHis balance is still the $of his unpaid balance for his visit on August 23rd.Due to his dissatisfaction with our office and the fact that he did not want to do the work that the doctor diagnosed, I called his insurance company and asked them to assign him to another officeIt was my hope that they could find a dentist that the patient would be happy withThe insurance said that they would transfer the patient to another office.I am enclosing a copy of our contract fee schedule with h is insurance planThese are the fees that we must followIf we change these fees we would be in breach of our contract with the insurance company and could be dismissed as a contracted provider at a minimum.Thank you,*** ***Office ManagerElder Creek Dental Group

I was having problem with my 1 tooth, it was little sharking, Elder Creek Dental did a x-ray and told me that "I need a have a root canal with specialist" Reception called and made my appointment with specialist I paid $595.00 in total for root canal. I never complained about my any other tooth but they take x-ray of other teeth after one week later I went back to have filling on root canal tooth. I told reception that "i'm here to get my tooth filling which I have root canal done because any time eat food it get stuck in. I don't know why they left that tooth and did filling in other 4 teeth when I wasn't having any problem. Once again i'm telling you with my honestly and very truthfully words they make me sign the agreement and charge me $259.00 for 4 teeth. When they told me they done then I talk to my wife they haven't did the filling on that tooth that I'm here for. We complain to dentist. Reception cam to us in room and told me that it will be a different charge for that tooth $125.00 to buildup and she make sign another agreement. I told the reception that's why i'm here for, so they fill that too same time after they done we went to reception and the total amount she told us $384.00 which my wife paid them with her card right on spot. They ask me have crown I told them not right now may be later because I expend too much money already. After one week later I went to dentist to get one of my tooth pull and they charge me $155.00 plus $5.00 for office visit. I paid them right on the spot and asked them that all my bill is paid for my portion, she told me and wife you are good, you paid all your share. After one month later I received a bill than I call them I paid all bills and you people told me everything paid then why sending me bills and what kind of charges is this, I talk to office manager and she started getting rough on me telling me to go find other dentist don't come back here I told her I will complain to Revdex.com which I did.
