Had to discontinue program medication because of adverse effects, company said they would do partial refund. They have not done so yet.
I have been attempting to resolve this situation since last year Sept/Oct 2019:

I signed up for a testosterone program with Elevate Men's Clinic in July of 2019. It was supposed to be for a 13-month period (see contract) and included office visits and medication, for a total of $2828, which was financed by the GreenSky Company. Shortly after starting the program (about 2 months in), I had a serious side effect from the medication- my blood pressure went up to dangerous levels. I was in my regular Dr's office and they became very concerned when the pressure was so high they were unable to get an accurate reading, there was actually possibility of stroke .(Later testing at home was still very high, 230/193). Testosterone has been known to cause blood pressure issues in certain individuals...I immediately stopped using the medication upon advice from my Dr. and within a few days my BP was back to normal levels. I then notified Elevate and THEY stopped the medication shipments at that time.
After several calls to Elevate, requesting a partial refund, I sent them a doctor's note and my medical record as they requested, and they said they would review the issue and get back to me. I didn't hear back so I called GreenSky. After GreenSky got involved, Elevate offered only a $750 refund...then tentatively offered $1000. However, no release document was ever sent to me and I also felt the refund should have been considerably more than just $1000, since I only had 2 office visits and just 2 shipments, out of a 13-month agreement. I then requested an itemized bill from Elevate so I could see the actual charges and they stopped communicating. I called at least 5-6 more times, (was hung up on once) left messages and was told they would get back to me, but no one ever called.
At this point, I have received neither the services nor product from Elevate as per the contract, so they are in breach of said contract. They will not issue a reasonable refund and will not communicate with me either. GreenSky attempted to resolve the issue but they no longer have Elevate Men's Clinic as a client so they haven't been able to do much.

Desired Outcome

I realize I should be responsible for some of the charges, but I was only in the program for 2 months and just received 2 shipments and had 2 office visits/bloodtests total. I was told that the $2828 amount would cover ALL medication shipments, office visits and labs for a 13-month period. Since they refuse to send me an itemized bill I can only estimate what the refund should be. The $2828 figures out to be approx $218 per month - 2 months would be $436. Subtract that from $2828 the refund should be around $2300. I would be willing to accept $2000 or the actual amount I incurred in the program.

On 7/22/20 Mr. was initially seen in our office and diagnosed with low testosterone. He purchased/financed a 1 year treatment plan. On that inital visit, lab work was performed as well as an office visit, followed by visits on 7/25/19 and 9/9/19 at which time additional medical equipment and medication were ordered and lab work and office visits charges were incurred. After receiving a letter from his physician stating his current medical condition and recommendation, he stopped his treatment on 11/21/19. Thus, an internal breakdown of all costs incurred to that date was in excess of $1800.00. A refund was offered to the patient in the amount of $1000.00 to which he refused. He attempted to dispute the charges with the finance company and was refused, but we still offered a refund in good ***. To date, the patient still refuses the refund settlement.

Why was this case closed? The treatment was for Dr visits, lab work and medication over a 13 month period. I was only in the program less than 2 months. There is no way the charges incurred were anywhere near $1800!Yes, Elevate offered the $1000 over the phone but did not send any kind of documentation to that effect, no breakdown of charges, nothing. I told them that was not a sufficient amount. and to let me know what costs I actually incurred. I requested but NEVER received an itemized bill for services rendered. They made NO effort to contact me with that information after many phone calls. It is not unreasonable to know what I am being charged for - Elevate has so far done very little regarding this issue in "good faith'. I do not consider this cased 'resolved' by any means.

We have offered a refund to the patient, but he continues to refuse it. This refund amount reflects the
unused portion of the medication and lab work appointments. The patient can either accept this refund or chose to become compliant with his care and
continue treatment once he is medically cleared to do so.

Called Elevate again last week, requested itemized bill, no response. I will settle for an $1800 refund, as soon as I receive the check I will close the Revdex.com case and withdraw my complaint with the *** office.
