Hello,This patient is being hostile for no reason We have signed a payment plan contract which she does have a copy of Her credit card has been changed so she can conveniently continue to have treatment without paying We have been more than accommodating because she said she lost her job We told her we would have to pause the treatment and not stop until she can continue to pay down her balance At this point she has been hostile, posting negative reviews and threatening to my staffShe last time showed up at the office and refuse to leave the premises if she did not receive any more of her trays This is not behavior of a mentally stable person We have made all attempts to be reasonable It is best she find another Doctor to transfer her case to I offered to do this transfer free of charge No cost to her and mail the remaining aligners to her new Dentist She has to fill out a Transfer Form when a Doctor will accept her case It is pretty simple to resolve

Dear [redacted] ,Thank you for expressing your concerns I have reviewed your chart and radiographs I do caution you against using words like Fraud and Malpractice The advertising clearly states that the Consultation is Free if you have Dental Insurance As it turns out, your Insurance was either not valid or didn't cover the Exam and Radiographs due to frequency You are responsible for Slander and Accusations will never be tolerated However, in order to promote an amicable resolve to the situation, I have waived your charges and you are free to get the Treatment you need at another Health Care Provider If you need your records, then you will need to pay for your Balance for the time and examination you had with our office I hope this resolves the matter, Warm Regards, Dr.S***

I would just like her to sign the Release and send her case to her at the most current address. It is best she find another Doctor to finish her treatment. Hello,I sent check number [redacted] from [redacted] Account ending in [redacted] on 2/7/17.  Her Fiance cashed his check only on 3/10/17 and her's is still to be cashed.    I would give it 2 weeks then I will stop it and resend another one.  We mailed it to the Address on file for [redacted] which is in Leesburg, VA.  Perhaps I would mail the other check to another address if that would work better.  Thanks,Dr.Anne S[redacted]

Review: My wife and I went for a routine cleaning at Elite Smiles Dental located at [redacted] in [redacted], VA. My wife's account with them had been overpaid because each time she received services in the past, they always demanded that she pay for the services before insurance would be submitted. On April 8, 2013, my wife’s cleaning was paid using the credit on her account. They then refused to clean my teeth because they claimed I had an outstanding balance of $348.70. I questioned this balance and after many phone calls and conversations, it was uncovered that the dentist office failed to properly submit insurance claims to our secondary insurance back in 2011, which we did not know about until that day in August 2013. This was properly documented using EOBs and Ledger Statements provided by Elite Smiles Dental. Because two years had passed we were unable to file a claim to our dental insurance companies. Since there was a credit on my wife’s account, they then transferred $178.86 to me. As a "professional courtesy", they applied $169.84 to my account which is the amount that would have been paid by the insurance claims that weren’t submitted two years prior. They cleaned my teeth on August 15 and I paid the balance they said I would owe after insurance to zero out the account. Soon after that visit we received an EOB from our insurance company [redacted], but it wasn’t [redacted]. The dentist had billed [redacted]. We called [redacted] and they said to contact the dentist and have them resubmit the claim to [redacted] as [redacted] will not pay the claim. Elite Smiles Dental said they would resubmit the claims. At around 1050 on May 1, 2014, my wife was on a phone call with the front desk receptionist at Elite Smiles Dental in response to a bill that we had received yesterday that was over 90 days past due for $63.00. This was the first bill we received. She told us we had to pay the $63.00 to the dentist and submit a claim to our secondary insurance. Upon calling the secondary insurance company, [redacted], they informed us that per their contract with the dentist office, the dentist is required to submit the claim and definitely not the patient. The money would be paid directly to the dentist. We also called [redacted] and they verified that a claim was never submitted for the August 8 visit. We called Elite Smiles Dental back to ask them to file the claim to the insurance company per their contract. They are refusing to do so and stated that any money that is paid to them by insurance will simply be deducted from their previously applied "professional courtesy” and that we would always owe them $63.00 regardless of any insurance money they are paid. Before rudely hanging up on us, she told my wife that we were going to pay the $63 or go to collections. Another issue is that they also refuse to supply an itemized statement of charges. The reason they used for not submitting that claim was that they used the credit on my wife’s account to pay for her dental cleaning and they will not resubmit the insurance claim. Truth be told, if they would submit my wife’s claim, then it would cover any charges that we are supposedly 90-days late for and possibly even owe us a credit. Since they are refusing to submit insurance claims or supply a statement of charges, we cannot bill out secondary insurance for both services. The result would certainly end up in a credit in our favor, which they don’t want to happen. Instead, they are going to send me to collections, which is unwarranted.Desired Settlement: First I do not want to be sent to collections. I would like Elite Smiles Dental to resubmit the insurance claim to [redacted] for the services my wife received on August 8. Then I would like Elite Smiles Dental to submit insurance claims to our Secondary insurance [redacted] for services provided to myself and my wife for the August 8 and August 15 visits. Then I would like Elite Smiles Dental to properly adjust my account and send me a check for the credit that I am due. Finally, I want the office staff to formally apologize to my wife for being so rude to her during a phone conversation that occurred on the morning of May 1, 2014, during which Elite Smiles Dental hung up on my wife.
