Complaint: [redacted] I am rejecting this response because: Elite Theraphy did not address any of the concerns I brought to the attention of the Revdex.com such as why I was billed for sessions when only were documented in my son's folder therefore flasifying visitsThey also did not address the fact that they told me my insurance would cover services when my insurance denied payment in February and I was never notified and they continued to see my son until May knowing my insurance was not covering services and instead sent me a bill on May 11th for $I am an intelligent person and to simply say insurance is confusing is unacceptableThis company is not honest and they mislead clients by telling them services are covered by insurance only to send them an outrageous billI never told [redacted] the services my son was receiving were wonderfulIn favt I told her I saw no difference in my son's speechAnother statement by this company Regards, [redacted] ***

8/3/Elite Therapy Center Academy for Speech & Language, LIC [redacted] Waco, Texas [redacted] Revdex.com Corporate Office La Posada Drive Austin, Texas Re: [redacted] To whom it may concern; I am writing In dispute of the complaint filed with the bureau and appreciate the opportunity to do soOur clinic has not misrepresented information to this customer and have attempted to compromise with her regarding this issueUpon initial evaluation, the parent provided her signature that she read and understood our Intake packetThe packet included a consent form, insurance and medical release form, and insurance benefits assignmentjfinancial agreement formThe explanation of benefits are quoted to the parent as a courtesy which was acknowledged in the intake paperworkClaims are determined by the individual's insurance upon receipt and the agreement states that if the benefits are different, the individual is responsibleSatisfaction with our therapy services has been expressed on multiple occasions by the parent both verbally and through continued attendance of >sessionsQuality therapy services were provided by a licensed, certified speech pathologist and should be paid for as agreedCommunication and explanations about insurance terms and procedures have also been providedAdditionally, we have offered options including payment plans & grantsUnderstandably, insurance is confusing; however, insurance policies as well has our paperwork arc very specific that the responsibility of the policy holder is to understand their benefits with their insurance providerIt is clear that the services are billable and we have acted in agreement with our contract with both the parent and the insurance companyIn addition to our paperwork that has been provided to the parent regarding the account balance from Academy for Speech & Language, LLC; the parent received EOBs, as mandated by law, that provided clear details about the patient's amount owedI respectfully request that the insurance and financial documentation be reviewed and considered in regard to this complaintThis provides evidence that the parent was informed and aware of the financial obligation and this complaint should be dismissedThank you for allowing us the opportunity to respond and defend this complaintSincerely, [redacted] ***M.S., CCC-SLP Speech Pathologist Director/ASL co-owner

To whom it may concern: Thank you for providing me the additional concerns regarding this caseBased on the communication with Mrs*** following the initial complaints, it is evident that we disagree on the facts of the case (see attached)Based on that, we respectfully request that any other communication occur with our attorney, *** *** ** *** *** *** *** * *** LLPMrs*** has been provided Mr***'s contact informationIn response to the most recent complaint, I have addressed each of Mts***'s concernsEvery claim billed to UHC has a corresponding speech therapy note in compliance with all, regulatory agenciesThese notes can be provided to Mrs*** upon requestThe communication folder we provide parents is simply an effort to increase the correspondence with parents regarding their child's therapyIt is not required which is why Mrs_ *** does not have a communication logged for each sessionThe folder contains a cover letter, (see attached), signed by therapist(s) that states thi.sVerification of insurance is a courtesy to patients and is not a guarantee of payment, this was also statedSpeech therapy services were discontinued immediately upon request of termination by Mrs***We have documented communication between the treating speech-language pathologists and Mrs _*** that does not indicate dissatisfaction with therapy servicesNor have I ever indicated she reported "the services her son was receiving were wonderitir In fact, if Mrs_ *** would have communicated that she was not satisfied with our services, we would have addressed it immediatelyAgain, we are sorry for her experience as this is the exception and not the norm since our establishment in We are hopeful that resolution can come to this case and continue to offer options; however, we must request that the communication, in order to be productive, is with Mr***

Quality therapy services were provided by a licensed, certified speech pathologist and should be paid for as agreed. Communication and explanations about insurance terms and procedures have also been provided. Additionally, we have offered options including payment plans & grants. Understandably, insurance is confusing; however, insurance policies as well has our paperwork arc very specific that the responsibility of the policy holder is to understand their benefits with their insurance provider. It is clear that the services are billable and we have acted in agreement with our contract with both the parent and the insurance company. I am rejecting this response because: Elite Theraphy did not address any of the concerns I brought to the attention of the Revdex.com such as why I was billed for 33 sessions when only 26 were documented in my son's folder therefore flasifying visits. They also did not address the fact that they told me my insurance would cover services when my insurance denied payment in February and I was never notified and they continued to see my son until May knowing my insurance was not covering services and instead sent me a bill on May 11th for $1980. I am an intelligent person and to simply say insurance is confusing is unacceptable. This company is not honest and they mislead clients by telling them services are covered by insurance only to send them an outrageous bill. I never told [redacted] the services my son was receiving were wonderful. In favt I told her I saw no difference in my son's speech. Another false statement by this company.
