Levi is a good trainer but when it comes to his "special promotions" you really need to watch out for the cancellation policy I signed up for a month promotion that started in October and was going to end DecemberI had to stop going in the middle of November because my son was having medical problems againI didn't go the month of December either because my son was having surgery for the 4th time and it takes a lot out of usSince signing the contract in October and all the family issues we had going on I didn't remember to cancel days prior to January's withdrawalHe didn't even reach out to find out why I wasn't going or to even remind me or ask if I was going to continueHe knows his contract that you sign will stick and he will get your moneyThe second that money came out of my account which was $I emailed asking him to please reverse itHis response was I could come back for just this monthI explained that I couldn't go back because my son is going to be starting chemo and radiation treatment and I have to be home to help with everythingHe didn't seem to care about thatSo just be aware of what you are signing or you will be out of money

Customer signed up for Personal Training with Elite Total Fitness on 4/3/14. Attached is Customers signed and dated Payment Authorization form, which clearly shows which program ($147 per month) Customer wanted to go with. Payment Authorization Form which Customer filled out and signed and dated,...

clearly states that "Client hereby authorizes Elite Total Fitness to charge listed credit/debit card on a monthly basis in the amount corresponding to my selected program UNTIL I CANCEL." Client claims she "did not give verbal authorization to withdrawal funds". Elite Total Fitness does not work on Verbal Authorization, which is why we have Customers fill out the attached document, giving written Authorization. Cancellation Policy which the Customer signed, dated and filled out clearly states, "Client must give notice of cancellation via email at least (5) Five days prior to the scheduled monthly debit date. Cancellation requests received after this deadline will be charged in full for next months services."Attached is Customers email to Elite Total Fitness wishing to cancel service on 7/19/14, 16 days after her monthly debit date, which Customer agreed to in the signed and dated Payment Authorization Form. Customers account has been cancelled and will no longer be debited for any additional funds, but Elite Total Fitness does not feel we need to refund any funds, per the signed and dated Payment authorization form, that Customer filled out.

I didn't go the month of December either because my son was having surgery for the 4th time and it takes a lot out of us. Since signing the contract in October and all the family issues we had going on I didn't remember to cancel 5 days prior to January's withdrawal. He didn't even reach out to find out why I wasn't going or to even remind me or ask if I was going to continue. He knows his contract that you sign will stick and he will get your money.
