Review: I received service from City of New Orleans Emergency Medical Service[redacted] (telephone number ###-###-####)on 3/26/2013. I have contacted the company by phone and mail numerous times to advise the company to file a claim with my insurance company. As of today, the claim has not been filed with my insurance company. Around 5/26/2013 I received a statement from City of New Orleans Emergency Medical Service company via mail from United States Postal Service. I called ###-###-#### which is the company's phone number listed on the statement in May or June 2013 and gave my insurance information and requested the company to file a claim with my insurance company. In 7/2013 I mailed the company a copy of their statement with my insurance information listed. On 7/3/2014 I called the number listed on the statement and found out the company is no longer in business, the phone number is disconnected, and another company is handling their billing. Therefore, I searched for the company online and I was advised to call ###-###-#### which is the phone number for. I called my insurance company to find out if the bill was paid and was advised a claim was not submitted. I called ###-###-####. Ms. [redacted] took my insurance information then said the account from New Orleans Emergency Medical Service was taken over by her company, she will forward my insurance information to the correct department, and someone will contact me by mail or phone.On 7/18/2014 I was advised by my insurance company via phone the claim from City of New Orleans Emergency Medial Service has not been filed. Therefore, the representative from my insurance company called ###-###-####. We were advised by Ms. [redacted] my account can not be found using my name, date of birth, and account number listed on statement. My social was not given to the driver I am concerned about this bill because I can't afford to pay and it needs to be filed timely in order for the insurance company to pay.Desired Settlement: I would like for the company handling the billing for City of New Orleans Emergency Medical Service to find the account number that they use (this number was never given to me)and file with the insurance company. If it is to late to file a claim, I would like for the company to not hold me responsible for payment. Also, my last name is the only name listed on the statement. The ambulance driver was given a copy of my insurance card when I arrived at the hospital.
