I was referred to Encore Rehabilitation by the two physicians that treated me after a auto accident which occurred 04/03/
I was treated there some times for injuries to my left had, and subsequently for rehab after surgery on my left foot
During my treatment at the Hartselle, AL location, I was presented with copay charges and charges which Encore deemed appropriate All of these charges were paid upon demand by debit card
Months after the treatment had ended, I began receiving bills for additional charges I have contacted both the Hartselle location where I was treated, and the the Decatur location which is threatening me with credit action I have asked each for an itemized reason for the additional charges To date, 06/06/2016, those items and or reasons have not been provided
They have continued to send the original statement showing an additional charge of for reasons unknown
