We are in receipt of a complaint filed to the Revdex.com regarding ID# *** .In response, we as a large medical practice
receive many requests for medical records. We do have systems in place for the receiving and sending of medical records. All documents are time stamped as to when we receive them either by fax or other means. Once received our medical records must respond by 7-business days or sooner if needed emergently. We have to follow law in this matter and have a signed release. We did receive the request by fax on 9-30-14. The sending of the records is also recorded. I have attached both of these time sensitive documents with this response. As you will be able to see we received the request for records on 9-30-at 2:55. The records were date and time stamped out on Oct 6, at 5:02pm. This follows our office protocol and there is no way the dates and times can be altered in our electronic medical records by staff. Every movement by staff is logged into the system. Meaning, if staff did not follow protocol we would know it. We appreciate the concern from our patient.
