On 12/13/the patient presented to us with a request from her general dentist to evaluate and treat tooth # A root canal was previously started on this tooth by another dentist and there was separated filling material beyond the apex of the tooth Root canal therapy was performed on tooth #and the patient was advised that the filling material from the previously attempted root canal in her gum was inert She was advised that if she becomes symptomatic, she must call us so we could perform another procedure called an apicoectomy to retrieve filling material The patient was then given written instructions to return to her general dentist for a permanent restoration of her tooth and we would send her a recall card to assess the healing of tooth # A letter explaining root canal therapy and was completed and a permanent restoration was necessary was sent to the patient's general dentist On 1/24/the patient came in for a check up on tooth # Everything looked and felt fine but the patient was again advised that she would need a buildup (final restoration) by her general dentist in tooth #and we would see her for a month recall All of our literature states that we do not restore teeth permanently and that patients must return to their general dentist for that procedure The patient was again advised to come in for a checkup in April so we could assess the healing of tooth #10.The patient stated that on 9/22/she had the too and foreign object removed by an oral surgeon We are unsure as to why the patient had her tooth extracted The patient never made our office aware of any symptoms she was having and never returned for her month checkup after the root canal was performed We could have helped her save the tooth with the apical procedure, as mentioned in her initial visit As far as the oral surgeon stating it was the worst root canal he had ever seen, it was in reference to the separated filling material in her gum which was done by a previous dentist In response to the patient's accusations that DrH [redacted] refused to keep his promises, DrH [redacted] offered the patient information regarding implants but never offered to arrange her appointments DrH [redacted] also never promised to do the permanent filling The patient was advised several times to return to her dentist for that procedure We are not refunding the patient's payment for the root canal We treated the patient with the highest standard of care and the loss of the tooth was due to patient neglect
