I'm responding to your email directed to Paul [redacted] dated January 5, 2015. We have no record of receiving anything from the Revdex.com prior to the above referenced date.
I understand Ms....
[redacted] confusion with the billing. A review of Ms. [redacted] account shows the following: Services were rendered to [redacted](son) On Jan 6, 2006, June 3,2006, Sept 15,2006,Jan.21,2008, Sept23,2009, Oct. 7,2009, Aug 25,2010, Oct7,2010, April 1, 2011. Our records indicate statements were being sent to Ms. [redacted] at [redacted], Inver Grove Heights, MN. 55076. During that time frame [redacted] turned 18 on May 3, 2012. He continued to receive services at our Inver Grove Heights clinic. And since he was no longer a minor he was setup with his own account. Due to a clerical oversight [redacted]s' services incurred when he was a minor were moved from his mothers account to [redacted]s' account, which is why his mother stopped receiving statements. The clinic corrected the oversight April 2015, moving the charges back to his Mothers acct. (who is responsible for services rendered when he was a minor) and she started receiving statements. A letter was sent to Ms. [redacted] on April 25, 2015 notified her of what had occurred. Copy attached.
Due to our error, causing statements to Ms. [redacted] to stop and the age of the balances, we are writing off the charges and statements will stop. I'd like to apologize to Ms. [redacted] for the confusion we caused her and not being able to get a clear understanding of what has occurred.
