payment. We asked for bank statements showing this activity. She produced statements with several different bank headers, these were copies of cut and pasted statements. The documents seemed fake. In response to Mrs. [redacted] continued complaint that we did not follow through on our end. She never offered to bring in the original documents, which I requested she do, She could have redacted any unrelated information from the bank statements, on the advice of our bank, what she submitted looked fraudulent, and we never had any record that the payments she insisted we took were ever made. There was on payment, which we did resolve with her. 
Unfortunately due to HIPPA constraints I cannot discuss her non-compliance with our program. I can say that the Dr. felt that continuing to treat her with the level of non compliance she had could be detrimental to her health. If Mrs. [redacted] would like to give her consent I am happy to share emails which support our reason for cancelling her program. In addition we are researching her account to see if she is due a refund. Please note that we gave her credit for several payments which we have never received, and when she did finally pay us, it did not even bring her current.

Went for a $99.00 special to check testosterone & hormone levels. Actually they charged $299.00. Could not get a good blood draw, because my vein collapsed. It stated that the quantity was not sufficient for analysis. Instead of doing a correct blood draw, they tried to sell me a program for $599.00 or 154.00 monthly. I am totally dissatisfied and I feel I been taken advantage of and never received anything, paying $299.00. Jenni Falk

I did not supply my original bank statements because I did not want to divulge all my financial transactions history to EnVoque. My credit card was compromised by someone using it illegally and I had to cancel my credit card and get a new one issued from my bank.  This happened on more than one occasion while undergoing treatment at EnVoque.  This is why EnVoque encountered some difficulty with the payments.  I explained all this to Envoque.  I told Envoque I would sit down with Kem and go over all the original bank statements and she did not want to do that.  Kem said she would go back and look at everything on their end and get back to me, but she never did.  Instead, I received a letter of cancellation from Envoque stating they were cancelling my contract after I paid through the current month. As far as not complying with the doctor's orders, he told me to see a endocrinologist, which I did.  He also told me to adjust my hormone dosages based on the symptoms I was having.  The doctor would not return my phone calls or emails when I was trying to explain my health issues with him regarding the hormones.  I called the office and the receptionist told me the doctor was not going to return my emails or calls because Kem had placed a hold on my account and services because of the credit card issues.
