The visit was billed according to the standards of the A[redacted], your insurance accepted the charges and assigned them to your deductible.
You called our billing office on June 16, 2015 and your were informed of the above. Per your request, we contacted your insurance company and we were advised by the [redacted] representative that the charges were assigned to your deductible.
On July 7th, our billing office spoke with you again and we offered a $1000.00 discount reducing your outstanding balance to $305.00 as a courtesy.
