no one has responded to me to follow up on this. I was told they would call me back and no one every did. Our account still shows multiple accounts and I have signed HiPPA form. Maybe no HIPPA for all (each and every) accounts you have under my name, but I have signed one.

I received your letter dated 7/9/2015 regarding a complaint submitted by you from the office of the Revdex.com of Minnesota and North Dakota We appreciate the opportunity to review and respond to these concerns.
Per your letter, you state that you would like your account closed with States Collections Services and a statement in writing with the date assuring that your credit report will not be affected in any way. You also state that you would like all of your accounts to be under one account number as it is confusing for you when you are making payments.
In my review of your account, I show that our department called you 10 times from 1/14/15 - 5/27/15 to discuss payment arrangements on the balance owing as well as sent 3 letters. We received no response to the calls or letters. A payment of $25.00 was received 6/8/15 which was below the clinic guidelines for a payment plan so the balance was referred to States Collections Services on 7/2/15. The balance of $222.01 will remain with States Collections Services.
