July 15, Revdex.com [redacted] Ave., 4th FlCleveland, Ohio 44115- To Whom It May Concern: I am writing in response to the complaint from the individual, ID [redacted] , who had damage to her vehicle that occurred on 7/5/ Approximately around 10:00am on July 6th we received a phone call from the individual stating that one of our vehicles hit debris (tire pieces) in the roadway which flew up and damaged her vehicle The employee she spoke with wrote down her contact information and told the individual someone would be in contact with her I was giving the information, which in turn I emailed our insurance company at 10:29am At 12:02pm, the individual with the damaged car then sent an email to [redacted] Company stating she wanted to talk to someone about the damage to her vehicle At that time I then called the insurance company (because I still had no response to my email) and received my agent’s voicemail I then called our other agent within the company and received his voicemail as well I left him a voicemail that I needed him to call me in regards to the incident I was finally able to make contact with our agent after hours, due to the fact that he was in meetings when I tried contacting him earlier He was not in the office at the time we spoke and he told me he would call me in the morning to discuss I then called the individual with the damaged car and explained to her that our agent was in meetings all day and I did make contact with him and they would be in contact with her in the morning The next morning, on July 7th, the agent contacted me and said he spoke to the individual and explained to her that they believe [redacted] Company did not show negligence in this case, it was an unfortunate incident and [redacted] Company should not be responsible for debris on the roadway Our agent advised her to contact her insurance company and if her insurance company felt we should be responsible for the damage, then there is a subrogation process in which her insurance company would contact our insurance company and the insurance companies would figure out a resolution for the damage to her vehicle As of today, 7/15/no one has contacted our insurance company Sincerely, [redacted] M P [redacted] Controller

on July 6th we received a phone call from the individual stating that one of our vehicles hit debris (tire pieces) in the roadway which flew up and damaged her vehicle.  The employee she spoke with wrote down her contact information and told the individual someone would be in contact with her.  I was giving the information, which in turn I emailed our insurance company at 10:29am.  At 12:02pm, the individual with the damaged car then sent an email to [redacted] Company stating she wanted to talk to someone about the damage to her vehicle.  At that time I then called the insurance company (because I still had no response to my email) and received my agent’s voicemail.  I then called our other agent within the company and received his voicemail as well.  I left him a voicemail that I needed him to call me in regards to the incident.  I was finally able to make contact with our agent after hours, due to the fact that he was in meetings when I tried contacting him earlier.  He was not in the office at the time we spoke and he told me he would call me in the morning to discuss.  I then called the individual with the damaged car and explained to her that our agent was in meetings all day and I did make contact with him and they would be in contact with her in the morning.  The next morning, on July 7th, the agent contacted me and said he spoke to the individual and explained to her that they believe [redacted] Company did not show negligence in this case, it was an unfortunate incident and [redacted] Company should not be responsible for debris on the roadway.  Our agent advised her to contact her insurance company and if her insurance company felt we should be responsible for the damage, then there is a subrogation process in which her insurance company would contact our insurance company and the insurance companies would figure out a resolution for the damage to her vehicle.  As of today, 7/15/16 no one has contacted our insurance company.   Sincerely,       [redacted] M P[redacted] Controller

I spoke to our agent last week.  He received a voicemail from the agent.  Our agent returned the call within 1.5-2hrs and got the agents voicemail.  I received an email from my agent today...see copy of email: From: Kevin O[redacted] [mailto:k[redacted]@alphapandc.com] Sent: Monday, August 01, 2016 11:54 AM To: [redacted] Subject: [redacted], As previously discussed I left a message for [redacted]’s [redacted] agent last week and did not receive a return call.  I called again this morning and spoke to someone, but the agent is on vacation this week. I asked if they had filed a claim or spoke to [redacted] about doing so, but she indicated there were no notes.  The agent is supposed to call me next week when she returns to the office.  Just wanted to give you a heads up.  Kevin M.
