Both [redacted] and her husband [redacted] had appointments scheduled for Saturday January 21, 2017 and both appointments were canceled without 48 hrs (office policy that was signed by [redacted] and **). As courtesy, appointments were re-schedule without $75 per hour charge...

for Saturday cancellation. We re-scheduled them to Saturday, March 11, 2017 - both appointments were missed so we had to charge each patient a $50 broken appointment fee. On Saturday, the broken or missed appointment fee is actually $75 per hour. Eureka Dental Group has paperwork that patients' consent to and acknowledge about policy and fees. Patient was not sent to collection right away. We sent them bills in April, May, June and nobody attempted to contact us to resolve the issue. Eureka Dental reserve time for each individual patients to provide the best dental experience and would greatly appreciate respect in return. I scheduled my son for a new patient appointment, gave insurance information and was told office would verify insurance before visit. At first appt my son had a full cleaning. Now received a bill because he was not due for cleaning. Office verified insurance but did not verify what was covered. I was told our insurance was too hard to understand what was covered so they went ahead and did cleaning. And now they are telling me insurance was verified. ..
