The patient scheduled and then received a refractive (Lasik) consultation, which is free The patient was not billed for this After that, the patient also scheduled and then received a cataract screening Cataract screenings are not free and the patient was told this We
subsequently billed the patient, and the patient refused to pay We strongly feel that the fair resolution here is that the patient pay their bill

This person is no longer a patient of ours, has already left bad reviews (in exaggerated fashion) about our firm on multiple review websites, and was simply calling to complain that she received a letter in the mail saying one of our doctors was no longer with us, a letter that was sent out to many
of our current and former patients In addition, she complains about our receptionist being less than polite, but she herself was very impolite to our employee, a fact that our recording of the telephone call she references will clearly demonstrate Furthermore, I spoke with her today and very politely apologized for the minor inconvenience of receiving a letter in the mail, so the main basis of her complaint is satisfied already Her demands that we retrain our receptionist are unreasonable and unnecessary, especially since she is no longer our patient, has stated clearly as of today she wants nothing to do with our business (a request we have assured her we will honor), and has even gone so far as to be borderline libelous to our company on multiple public review websites This claim is completely frivolous and should be closed immediately

I am rejecting this response because: Please see attached document to Evergreen. Today,

insurance company for treatment coverage, which is why they are providing
payments.However, the insurance company relies on your prompt and 
accurate submission of treatment rendered to me to provide payments.I don't see how this information has any bearing to this issue but
I am providing it non-the-less for your reference.To simplify the issue, the total cost of all treatment is
USD$6321.08.To-date, my patient responsibility per insurance statements is USD1,397.77.Since I have paid for all costs upfront, the difference between
the total treatment cost and patient liability are to be reimbursed back to me.Does this make sense? Let me know if you have any questions.To reiterate, all I am asking for is the difference between total

See attachment and letter which was emailed on 1/10/2018. Since letter was sent we have received response to appeal and Premera has made date of service 12/14/16 patient responsibility. The patient should have received a copy dated 1/19/2018 from her insurance company.  I have attached for...

Cataract surgery requires areplacement of the lens in your eye. The patient decided to pay extra for a premium lensinstead of the standard lens. This information was explained to the patient and she signed thepaperwork explaining that her insurance would not pay for a premium lens. Her insurancecompany did pay for part of the premium lens. Each time the insurance company sentEvergreen a payment, the patient was refunded the amount received.When the insurance company sent an estimate of benefits (EOB) form stating Evergreen had acontractual write off, our billing department resubmitted the claim. We explained that this wasfor a premium lens not a standard lens. The surgery date of 12/21/16, was reprocessed by theinsurance company and the EOB was sent to the patient showing this was her responsibilityand not a contractual write off. Evergreen has requested the surgery date of 12/14/16 bereprocessed also. The request was sent again today to the patient's insurance company 1/9/18.Evergreen does not owe any refund to the patient.
