Phone: ###-###-#### Extension 301 Fax: ###-###-#### February 23, Re: *** To Whom It May Concern, This is the first time in years of providing our transportation services that we have encountered this type of a problem The person at the rider's facility who normally books trips with us notifies us when a wide-ramp van is needed Unfortunately, this is not the person who booked this particular trip Referring to a wheelchair as 'Bariatric' only alerts us to its weight limit - not to the width of the chair In this case, the chair was exceptionally wide and would not fit on the ramp of the van we sent for this trip We did inform the caller that we had a van that could accommodate this trip, but it was not in the area at the time In order to assure we provide excellent service going forward, we have created a script for our call takers to use in which they ask for the width of the chair when making reservationsThank You, Joseph LL*** *** Executive Transportation LLC
