Jonathan S***, DMD FAACDExton Dental Health Group [redacted] ***Exton, PA ***ID # [redacted] Customer [redacted] Dear [redacted] , This e-mail is in reference to an original letter from you dated 9/16'(Complaint ID [redacted] ), and a follow up letter on 9'28/stating that you had not received a response A fax was sent to you (and confirmed as delivered) as per your instructions on 9/18/at 1:PM requesting an authorization to provide you with treatment details for the Jennings family as per HIPPA regulations We have not received that authorization and for that reason I am not at liberty to give you specific details concerning the family's treatment I can say that all of the requests in the original letter from the patient have been honored as an accommodation regardless of the complaint's merit I cannot specifically discuss the Jennings family with you without the information release authorization, but I will share with you the patient agreement (attached) in reference to the office appointment cancellation and no-show policy that ALL patients and legal guardians sign at the time of registration Although we are quite lenient in enforcing this policy, there is a point at which excessive schedule changes deprive other patients from having available appointments, and the cost of office staffing during cancellations and no-shows becomes unfair and unreasonable It is unfortunate but in some cases it is in the best interest of the patient and our office for the patient to seek treatment elsewhere Let me be clear that this response is a statement of general office policy-not specific to any patient or family, and I would be pleased to be more specific about the Jennings family's relationship with our office if we receive the proper authorization as required by law Sincerely, Janet S***
