June 1, To Whom it may concern[redacted] visited DrT [redacted] on March 3, for a annual eye examHis exam revealed that his eyeglasses prescription had not changed, however his contact lens prescription needed updating to a different powerHis medical exam portion of the exam revealed that he has significant issues in both eyesA medical filing was made with his United Health Care planUnknown to us at the time, our participation as a provider with United Health Care was put into out of network status when we left a group practice and started a sole practice[redacted] Contacted us when he received his bill for Out of network Service and we apologized for his additional fees when we discovered the out of network filingAt this point, we contacted our billing company to dismiss the additional charges to [redacted] believing all was taken care of Upon receiving the letter today from the Revdex.com we discovered that our wishes were not carried out by our billing company to write off the balance of the charges to [redacted] A phone call was placed today to change the balance to reflect a zero balance.Teresa C Office Manager

On December 24th, [redacted] came to our North Park Ave, Chevy Chase, Maryland office to place an order for glasses for her husband [redacted] Before placing the order we were informed that the prescription sent to us from another optical company was expired [redacted] and myself came to the agreement to go ahead and make the glasses with the expired prescriptionWe placed the order and ran it through their insurance which is [redacted] VisionBefore any insurance discount the total purchase amount was $for a Oliver People frame and polycarbonate, single vision lensesAfter their coverage was applied with [redacted] Vision, the copay they were responsible for was $The order was completed we sent it out thfhe insurance company and they manufactured the glasses from thereWe received the glasses on January 9th, from the [redacted] Vision Laboratory [redacted] came on February 5th, to pick up the eyeglasses and took them home with himA couple weeks went by and he called and said the prescription didn't seem to be correctWe offered him to come in and drop the glasses off so we could verify them to be sure they were correctWe found out that the were according to the prescriptionWhen we called the optical company where the prescription came from originally, they would not release it to us because it was expiredWe notified [redacted] of what was going and informed us that he would go and try and get a new prescription to send to us to remake his glassesAfter about weeks we didn't hear back from him, then his wife [redacted] reached out to us claiming we fraud to their insurance stating we billed [redacted] Vision to timesWe never billed the insurance twice only once and that was for the services rendered on December 24th,Her insurance company then contacted us regarding the situationWe came to the solution that we would refund them the copay of $they paid and [redacted] Vision would reinstate her benefits back [redacted] contacted the office manger on April 23, outlining her complaintAt this time a refund check was issued to [redacted] on that date and mailed the patient's home address the following day [redacted] Vision retract the money they paid to our office for the services we provided, and we would be able to reinstate the benefits used in once the [redacted] 's returned the eyeglasses back to usAs of 05/022/we still have not received the eyeglasses

I am rejecting this response because: I have already paid the disputed amount to avoid impacting my credit record. Once I receive a check with the disputed amount, or a refund is made to my credit card,  I will remove my complaint.  But my complaint stands until I receive the credit.

After their coverage was applied with [redacted] Vision, the copay they were responsible for was $150.00. The order was completed we sent it out thfhe insurance company and they manufactured the glasses from there. We received the glasses on January 9th, 2015 from the [redacted] Vision Laboratory. [redacted] came on February 5th, 2015 to pick up the eyeglasses and took them home with him. A couple weeks went by and he called and said the prescription didn't seem to be correct. We came to the solution that we would refund them the copay of $150.00 they paid and [redacted] Vision would reinstate her benefits back. [redacted] contacted the office manger on April 23, 2015 outlining her complaint. [redacted] Vision retract the money they paid to our office for the services we provided, and we would be able to reinstate the benefits used in 2014 once the [redacted]'s returned the eyeglasses back to us. As of 05/022/2015 we still have not received the eyeglasses.
