*** *** was seen in this office on October 1, for the start of her crowns on teeth #and #The crowns had open margins and decay underneathAt this time the patient paid $Her total estimated copay was $for the two crowns.
On
October 22, the new completed crowns on teeth #and #were permanently cementedThe fit and margins of the crown were good and the patient was happy upon insertionThe patient left however without making her remaining payment of $600.00, she said she would mail her payment in, November and December went by without a payment from the patient on her account.On January 19, 2015, fully months after the permanent insertion of the crowns on teeth #'s and #19, the patient came in to the office for an exam and X-ray stating that she was having trouble chewing on the crownsBoth #and #were adjusted at this time, DrCousins jet the patient know if she was still having trouble on these teeth she should have a consultation with the Endodontist to see if the root canals that were previously done by another Endodontist needed to have a retreatmentAt this time the patient paid another $200,on her balance of $She again stated she would send the remalning $payment in the mailOn January 27, 2015, the patient came back to the office stating she was unable to chew still and the teeth were bothering herDrCousins again examined her and said there was nothing wrong with either crownIt is the teeth and the previous root canals that need to be looked atThe patient's permanent crowns were put in temporary cement so the Endodontist would be able to remove them without damaging them to evaluate the teethWe referred the patient to DrRonald T***'s office for a consultation for possible retreatment of teeth #and #previously done root canalsOn January 30th, the patient presented to DrT*** office for a consultation regarding retreatment of these teeth, Dr, T*** told the patient she would possibly have to have these teeth retreated at that time if they were bothering herDrT*** also told the patient is was possible one or both of the teeth had an internal fracture and that could be causing the pain upon chewing as wellAt that time DrT*** recommended the patient have a CT scan done and then return with the results and for the possible retreatments, To date, the patient has not returned to DrT***'s office for any further consultations or retreatmentsNor has she returned to this officeNo data regarding a CT scan has been viewed either by this office or DrT***'s office to determine any possible treatment or diagnosisAt this time, the patient does have the permanent crowns In her mouthThey are not temporary crownsShe does need to go back to the Endodontist to have these teeth looked at for retreatments if they are still bothering herNothing was done In this office that was not correctThe crowns look greatIt is the root canals that need to be retreated by an Endodontist if they are still bothering the patientThis has been explained to the patient both by our office and by DrT***'s office several timesMy staff has spoke with the patient several times in the past few months letting her know she needs to have the teeth looked at for possible retreatments of the root canals if the teeth are still painfulThis is the only option that is going to alleviate the pain and or chewing sensitivityAt this time the patient's balance is $The dental insurance company did not pay her claim for the two crownsTechnically, the entire balance is the patient's responsibilityHowever, I would be willing to accept the remaining $within days by the patient to close out her account in finalityThank you for your attention to this matter, it is greatly appreciatedSincerely,
