On September 10,2014, the new patient [redacted] was seen at our camelback office for a $29 [redacted] /regular clean ing, exam and x-ray new patient special (reg $278). This special clearly states that any patient with periodontal (gum) disease, must have the proper periodontal cleaning.[redacted]...

had a tooth exam where her teeth were diagnosed for needed treatment. Aperiodontal exam was done as well, where the gums(gingival) are evaluated for gum disease. We evaluated her gums through a measurement process called periodontal probing. Her periodontal measurements demonstrated deeper gum pocketing and visually a heavy calculus(tartar) buildup including red, swollen erythmatous, bleeding gingival was around her teeth. [redacted] was given a full explanation of the treatment that was required. She agreed to the Fullmouth debridement (FMD) treatment before it was began by our office. Then she paid for the services rendered on that September 10, 2014. The one time [redacted] fee of $29.00 was applied and subtractedfrom her overall cost. On October 20,2014, our office called her to schedule her dental fillings, cavities, crowns, etc. [redacted] stated she would not be returning because our prices were too high and is getting dental workdone elsewhere. Her account was inactivated. [redacted] mentions deep cleaning. There are 2 types of deep cleanings. The first type is a 4 quadrant deep scaling and root planning, the fee is at $250 per quadrant.($1OOO.OO) The other type ofdeep cleaning is called a full mouth debidement ($175) that was done on September 10,2014.Even though [redacted] was inactivated, to our surprise she made another dental cleaningappointment on Febuary 27, 2015 at our other office on 51st ave. She came to the appointment andthought that the fee would be $29.00. [redacted] is for new first time pat ients only, of which she was no longer new, and of which her [redacted] had already been used on September 10, 2014. Other [redacted]s may be purchased as gifts, however they must be used on a new patient only. Prior to this cleaning, we made her aware of her cleaning fee of $93 .00, plus she purchased a bottle of medicated rinse for $16.00. She agreed. Later, after she received her cleaning and dental product was provided to the her,the balance was paid by credit card and signed.($109.00) Dr. [redacted]
