Mrs.[redacted]  came to our office on 12-2-2015 for an exam.at that time Dr.[redacted] had  diagnosed root canal and build up on Tooh # 23 and  scheduled an appointment  for that treatment on 12-19-2015and patient came in on 12-19-2015 got the Root Canal done on that...

day. I have attached proof of completed Root Canal and also the consent form she signed  for the Root Canal., and the xray shows the Root Canal finshed. If you have any question please  call me. [redacted] ( Ofc Mgr )

Correct/Revised claim were sent to insurance company (copy enclosed). Please let the member know as we will notify her that problem is solved?.

Review: I was billed for a root canal I never received. I also prepaid for a net set of partial teeth and was told my payment would cover the total expenses of work I needed done. They wrote paid in full on my bbilling statement however they are trying to charge me an additional $7,185 to complete the work.Desired Settlement: I also would like a refund on the root canal that was never preformed.

Review: My family and I were patients at Dr. [redacted]'s office for quite few years, we have Delta Dental insurance that started out paying 70% of our cost, paying 10% more per year up to 100%. We were aware that we were liable for the remainder of cost. We had payments taken out automatically from our checking. Due to discrepancies in billing, for example, double billing, billing for services not rendered, we are no longer patients there. We were, however, continuing to pay off our balance. At the end of February 2015 [redacted] in Dr. [redacted]'s office told me that we owed $178. I set up 2 $50 payments for March 2015 and asked if she would take out the remainder of $78 on April 7th 2015. The March payments came out but April 7th came and went and no payment was taken out. When I called to inquire why the last payment wasn't taken out I was told that I now owed $296. I have our "dental benefits statement" from Delta Dental. It shows that we do not owe $296. We owe $78. Also at one point an employee at the office told my husband not to have work that Dr. [redacted] recommended done because it was not necessary, that they just wanted more money.Desired Settlement: I would like someone from the Revdex.com to look into this. I am sure that we are not the only people they are doing this to. I would also like to hear back on the results of your investigation.

Review: On 10/8/2014, I recieved a letter of notice from Family Dental Group that my portion of the dental treatments was not paid in full by the insurance from date, 4/29/2009-2/19/2011 for $144.23. Which I was not aware of this bill until this note was recieved. I called the office and they recommended a payment. I feel that I am not obligate to pay this bill. Please help to see there's a resolution. Thanks for all your consideration and support.Desired Settlement: I am requesting to drop or waive this bill in the amount of $144.23.

Review: On 10/12/13 my family visited the Family Dentistry for our dental needs. After the appt. my wife asked [redacted] the office mgr for a treatment plan, we were told that they will contact our insurance company first with the services we need and will mail us a treatment plan. We never received any plans in the mail. I was told by Dr. [redacted] that I needed a crown and once my insurance company approves it they will call me in for an appointment. [redacted] confirmed that the insurance company will pay a portion which I knew was 90% and I will pay the balance 10%. Again they didn't give us any treatment plan in writing. On 1/20/14 I went in for my appointment thinking that the crown is covered as I was told. On 3/11/14 I received the dental benefits statement from Delta Insurance stating that the crown is not covered as it was worked on 4 yrs ago. Dr. [redacted]'s office NEVER contacted us to tell us that the crown is not covered and neither did he mention that this tooth was worked on before. As we had a credit card on file they began automatic payment. When I inquired with delta dental and my previous dentist, I was told that if I had gone back to my previous dentist there would have been no charge for this work, however, I am grateful that my previous dentist agreed to pay for the crown. So my wife called Dr. [redacted]'s office & asked them for a final bill and also questioned why we were not notified of the insurance not covering the bill. A disappointing phone call she received from [redacted] on 3/27 and was told "That if the insurance does not cover it is not our problem it is your problem." My wife requested a bill as we did not wish to continue business hence we wanted to pay and close off the account. To our surprise on the final bill we were charged incorrectly for 3 Fluoride services by $133.20. When my wife called back on 04/1, she was again told by [redacted] that she needed to look into it. A week later today instead of receiving a corrected bill I received a letter stating that my wife was threatening them.Desired Settlement: I will like Dr. [redacted]'s office to do the following: 1) Correct my bill as requested to offset the Fraudulent charge of $133.20 for Fluoride services rendered to my kids on 10/12/13. The patient portion as per my dental benefits was $4.20 but their statement reflects $47.20.2) Remove the charge for the crown from my bill, as they failed to notify me that the service is not covered by my insurance before providing the service. 3) Make sure other patient's are not billed fraudulently.

Mr. Ali came in with a tooth ache and broken crown on tooth number 2. I explained to him that the crown on tooth number 2 is broken and I have to make a new crown to restore the tooth. Mr. Ali Sameer agreed to get a new fun porcelain crown on tooth number 2, and signed all the consent form. with his consent for tooth #2 I proceeded with the crown procedure on that tooth #2 and gave him a full porcelain crown..

I have had the most horrible experience with the Family Dental Group and will never return to their service. [redacted] Office Manager is the most unprofessional individual I have met at a dental office. They are not customer oriented but will reap you off. They will work on how they can make the most money out of an individual instead of putting the customer needs first. Biggest professional thieves I have ever met. They NEVER provide you with a treatment plan. If your insurance is paying for the services, they will easily increase the patient portion on your statement from something that is suppose to be $4.20 to $47.20. Will insist for you to sign up for a payment plan and will never send you a bill until you ask for it. If you file a complaint to adjust the bill, they will send you a 5 day notice or will threatened to send you to collections adding a $400 collection fee to your account. I will never do business with them and will not recommend anyone to, if you are with them please make sure to check your bills everyday.

Review: I was recommanded by my friend in California Department of Insurance that because I have no insurance coverage, I can not get assistance from that Department.

The Dental clinic Staff availed my treatment under Anesthesia to ask me to select the installment and to allure me to sign some paper which I thought to be the confirmation of installment but actually it was a credit card application.

The next day, I went to the clinic and got the treatment statement, I found the fraud. They too much over charged me so I sent them a letter with a money order of $233.XX to pay the service. But the credit card company does not accept my explanation. Still ask me to pay. Can you help in this case. Thank you. Attached is my mail to the D.D.S.Desired Settlement: Don't want the credit card.
